About your Bowel Movements

This article is about your bowel movements. Dr. Michael Camilleri is a consultant and professor in the division of gastroenterology and hepatology at the Mayo Clinic in Minnesota. He mentioned that in the Victorian era it was thought that a person should have a bowel movement every day. But now gastroenterologists agree that three bowel movements per week are satisfactory. If you don’t have that spontaneously, you are said to suffer from constipation. On July 12 CNN published a review about the same topic.

But other people who consume a lot of fiber may have a bowel movement two or three times a day. This, too is considered normal. But in addition to the frequency of bowel movements the consistency of it is very important as well. “The stool form, appearance or consistency of the bowel movement is actually a much better criterion than the simple numbers to frequency,” Camilleri said.

Bristol stool chart

The Bristol Stool Chart classifies stools into seven groups. With this in mind, the healthiest types of stools are types three and four. This is where the stool is shaped like a sausage with cracks on the surface or snakelike and smooth.

Experts say that if you are excessively straining during an attempt to poop or if you feel like you have not totally emptied after a bowel movement, you have to make changes. Either you need to poop more often or you need to get a healthier stool quality.

Your body posture influences the ease of your bowel movements

If you put your feet on a stack of books, this raises your knees above your hips. In that position your pelvic floor muscles relax, which supports your bowel. The end result is that poop passes through more easily. This advice comes from Dr. Trisha Pasricha, a gastroenterologist at Massachusetts General Hospital and instructor of medicine at Harvard Medical School. She commented further: “We did not evolve to have our bowel movements sitting with our hips at 90 degrees on a chair, which is what we do now. We used to all have bowel movements squatting. Sitting at that sort of upright, 90-degree angle actually closes off the passageway.”

Proper food intake necessary for a good bowel movement

Start with a Mediterranean type diet, which consists of a lot of vegetables, salads, fruit, whole grains and nuts. According to the FDA the daily fiber load should be about 25 grams or more. Kiwis and prunes are useful as a dietary fiber source. On the other hand, you don’t want to overdo the fiber intake as it can cause abdominal bloating and loose stools. A high fat diet slows your digestive system down. But caffeine and caffein containing beverages stimulate your bowels. In addition, regular physical exercise gets your bowels moving.

Constipation

Constipation is an important symptom that prompts patients to see their physician. There seems to be a lot of confusion in the general public about this topic. There are enormous cultural differences. For instance, in Africa where the population eats on average a much larger amount of fiber, the bowel movements are much bulkier.

Sir Dr. Burkitt, the famous English surgeon, examined bowel movements (stools) of African tribes in comparison to his English countrymen and came to the conclusion that in the Western world we need to remedy our constipation problem and cancer of the colon problem by eating more fiber.

What we know now

He is still right: fiber is mainly treating constipation (not preventing cancer of the colon), but the chemicals that are also in the vegetables contain a multitude of natural anti-carcinogenic substances, which provide the powerful preventative action against colon cancer and many other cancers. Lycopene is one of these. Tomatoes and tomato products contain it.

Sir Dr. Burkitt’s observation that high bulk food (with vegetables and green leaves) prevents cancer is as valid today as it was in the early part of the 1900’s.

Chronic constipation

Chronic constipation cases that start insidiously, but then remain despite taking a high fiber diet, make the doctor think about other underlying causes such as hypothyroidism (= low thyroid function) or metabolic causes such as hypercalcemia and uremia from early kidney failure.

We find that elderly people become too inactive. This lowers the natural peristalsis of the gut. In combination with poor eating habits and chewing problems because of poor teeth it is often responsible for chronic constipation. Also, elderly people often are on multiple drugs, all of which have a weak “anticholinergic” side effect, which translates into suppressing peristalsis chemically resulting in constipation. Psychogenic factors and chronic depression as well as obsessive-compulsive behavior can also feed into constipation. It can lead to a hyper awareness of one’s own bowel pattern. This is unhealthy and requires counselling. But first the doctor must rule out any serious cause of chronic constipation.

Treatment for constipation (simple steps that help)

As mentioned above, the patient has to modify the food intake to include as much fresh and steamed vegetables as possible. This will lead to bulk in the colon and make the stool softer thus allowing it to pass through the rectum and anal canal easier. The other advantage of this simple step is that the bulkiness of the stool triggers the normal peristalsis movement of the colon moving the contents towards and through the rectum. Decreasing the passage time in the colon allows less water absorption, keeping the stool soft and pliable until it leaves through the anal canal. This is easily achieved by a diet rich in fruits and vegetables. Cereals containing bran like slow rolled oats (not instant oats) are also useful when taken with breakfast. Cut down on meat and fat consumption.

Enema with lukewarm water

A simple tool for chronic constipation is an enema with lukewarm water (1500 ml or 50 fl.oz.) in a Faultless enema bag without any additives in it. The person who gets the enema needs to lay on the left side. This way the water can flow in easily into the colon. When entering the nozzle into the anal canal, do not force it. Instead use a bit of Vaseline ointment to facilitate entrance. The response usually comes within 5 to 10 minutes following the enema. It works by dilatation of the bowel wall, which leads to a reflex bowel contraction. Although it would be safe to take this every day, usually you should not have to take it more than two or three times per week even in chronic constipation.

Extra fiber (bulking agents)

Next, if this is not tolerated or does not appeal to the patient, I would recommend a bulking agent such as psyllium (brand names: Prodiem Plain, Metamucil, Novo-Mucilax) and bran (Brand names: Kellogg’s All Bran and Post’s bran flakes). These are mild laxatives, which are safe to take every day and which will not make the patient hypokalemic.

Osmotic agents

Osmotic agent: One or two tablespoons of sorbitol as a 70% solution is a hyperosmotic solution, which stays in the gut and draws water by osmosis into the colon. The bulking effect creates peristalsis and the water retention makes the stool softer. It takes often 1 or two days to get the full effect. There can be transient abdominal cramps until the stool is passed. One variation of this theme is to give sorbitol in a mix with other osmotic agents as a micro-enema in the form of Microlax (from Pharmacia and Upjohn).

The emergency break (laxative)

The “emergency break”: Occasionally all of the above is simply too weak. The constipated person normally is controlled with the above measures, simply could not go to the bathroom for several days. Bisacodyl (brand name: Dulcolax) and sennosides (brand names: Ex-lax Sugar-Coated Pills and Senokot products) can help you unload any held back stool on a one-time only basis. Increase your vegetable and fruit intake, cut down on refined foods and fatty foods, which are devoid of fiber.

Because of its role in nutrient absorption and body detoxification the digestive system plays a direct role in overall health and wellness. When the colon is unclean, waste and toxins can become trapped. This places an extra strain on the detoxification organs (kidneys, liver) and weakens the immune system causing illness. Eat Right for a Long Life

Dr. Felice Gersh gave a talk at a conference in Las Vegas stressing the importance to eat right for a long life. This was at the 27th Annual World Congress on Anti-Aging Medicine in Las Vegas from Dec. 13 to 15th, 2019. The actual title of her presentation was “Nutrition for Longevity”.
Dr. Gersh has a fellowship in Integrative Medicine at the University of Arizona School of Medicine.

In the first place she pointed out that an anti-inflammatory diet consists of vegetables, fruits, nuts, whole grains, healthy oils like olive oil and fish. This is a modified Mediterranean diet. On the other hand, with a pro-inflammatory diet or Western diet, you eat high fat, cholesterol, lots of protein from red beef, high sugar, excessive salt and a lot of processed and fast-food.

Prevention of diseases

It is important to realize that for prevention of cardiovascular disease, cancer and degenerative diseases you need to eat fruits and vegetables, which contain important phytochemicals. They contain a wide variety of molecules, like carotenoids, vitamins and polyphenols. Another key point is that cruciferous vegetables (broccoli, Brussels sprouts, and kale) contain glucosinolates, which are sulfur-containing compounds. They protect you from cancer.

Vitamins and magnesium

It must be remembered that in order to strengthen the immune system and prevent hardening of the arteries (atherosclerosis) we need the following: omega-3 and omega-6 fatty acids, the enzyme Co-Q10, vitamin A, B complex, C, D, E, carotenoids, phytosterols, stilbenes and flavonoids. Another key point is that magnesium is extremely important. Many of our dietary habits have reduced magnesium intake to a minimum. Our soils are depleted of magnesium, it is no longer in drinking water, and it is absent in processed foods. However, magnesium is involved as a co-factor in more than 700 enzymatic reactions in our bodies. Magnesium is involved in heart contractions, is important to maintain our blood pressure and is important for glycemic control. It is also important for bone development and for DNA and RNA synthesis. Magnesium binds serotonin and dopamine to their receptors and plays a role in many more body functions.

Brassica vegetables

This group of vegetables consists of broccoli, cauliflower, green cabbage and Brussels sprouts. Notably, they have long been recognized to lower the risk of many cancers. The first thing to remember is that the active ingredients in them are glucosinolates and isothiocyanates. Certainly, food preparation has a lot of influence on maintaining beneficial substances in the brassica vegetables. To emphasize, finely shredded vegetables had a marked decline of their glucosinolate levels by 75% within only 6 hours. On the negative side, microwave cooking destroys 74% of glucosinolates, but on the positive side, storage in room air preserves almost all of the glucosinolates for 1 week. In addition, stir frying brassica also preserves the glucosinolates. This reference points out how fruit and vegetables can contribute to cancer prevention.

The gut microbiome

The Western diet leads to a change in the gut flora with Gram-negative bacteria taking over the healthy gut flora and disrupting the intestinal barrier. To emphasize, this result is called endotoxemia. Part of this is increased serum endotoxin, which mainly consists of lipopolysaccharides. Indeed, it causes gut inflammation and a breakdown of the gut barrier. When this happens, autoimmune antibodies are produced. To put it another way, the Western diet undermines your health. In a word, high fat foods and added sugars (refined carbohydrates) lead to increased Gram-negative bacteria and the disruption of the intestinal barrier.

An unhealthy diet causes disease

In the long run this causes autoimmune diseases, leads to higher heart attack rates and to diabetes. Healthy gut bacteria in fact help to digest fibre, which leads to three short-chain fatty acids: butyrate, acetate and propionate. For the most part, they are important as energy source, affect cardiometabolic health and appetite. On balance, butyrate also helps to maintain the blood brain barrier.

Importance of fiber

Higher fiber content in food leads to less cardiovascular disease, has positive effects on obesity and the metabolic syndrome. Fiber changes the microbiome in the gut, leads to less gut permeability and more short-chain fatty acids production.

The best diet

  • 60% to 70% complex carbohydrates are the foundation of a healthy diet. It consists of vegetables, green leafy and root vegetables, beans, legumes, whole grains and fruit.
  • Eat healthy fats from nuts, olives, seeds, krill or fish oil. Limit fat intake to 18 to 28% of your daily calorie intake. Avoid hydrogenated fats. Limit your saturated fatty acid intake. 85% chocolate is OK. Otherwise consume olive oil and omega-3 fatty acid containing foods (from seafood and fish).
  • Add about 12% of the daily calorie consumption as protein per day. Choose fish, seafood, lean cuts of chicken, only the occasional red meat (organic or grass-fed meat)
  • Eat lots of fiber, eat organic and minimally processed food. Limit sugar, fat and salt. Avoid antibiotics from the agricultural industry, sweeteners, gluten and excessive alcohol intake. Take a daily probiotic and eat probiotic food. Eat three meals a day, a big breakfast, a moderately-sized lunch and a small dinner. A fasting mimicking diet once per month for 5 days activates your longevity genes.

A Mediterranean type diet gives you the right foods that you need for your health. Avoid the Standard American diet as it is unhealthy and kills the good gut bacteria. Non-Alcoholic Fatty Liver Disease

Non-alcoholic fatty liver disease (NAFLD) has been increasing in the general population. About 25% of all Americans suffer from this condition. It is more common among obese, overweight and diabetic patients. But it is also very common in people whose gut bacteria are abnormal. Antibiotic residues in red meat and in poultry can cause these gut bacteria changes according to scientists. Metabolic products of these abnormal gut bacteria are absorbed in the gut and end up in the portal vein. This is a large vein collecting all the nutrients and leaked toxins and transports them to the liver. The liver detoxifies toxins and digests nutrients. In the process the liver can get inflamed and turn into a fatty liver.

Fatty liver is still reversible

When liver damage continues, fibrotic changes can develop and liver cirrhosis is the next stage of this disease. Fatty liver is still reversible, but liver cirrhosis is not.

The January 2020 edition of LifeExtension magazine contains a review of non-alcoholic fatty liver disease (NAFLD). It also tells you what you can do to normalize the condition. In the following I am reviewing this article.

Probiotics can help to reverse non-alcoholic fatty liver disease

Two studies that I will describe in more detail later showed that probiotics can reduce inflammation in the blood significantly and can reverse fatty liver disease. Abnormal bacteria in the gut can also lead to leaky membranes of the gut wall. This can become permanent and the condition has the name “leaky gut syndrome”. Researchers have linked this condition to the development of autoimmune diseases. People with obesity and with high blood sugars (people with diabetes) have the highest risk of developing non-alcoholic fatty liver disease (NAFLD). The condition (NAFLD) does not produce any symptoms. But a combination of blood tests and scans can diagnose the condition.

Good and bad gut bacteria

If you allow the bad gut bacteria to stay, inflammation of the gut wall with leaky gut syndrome develops. This allows bacteria, toxins and other substances that should stay out of the blood to enter the body. Researchers have found that good bacteria can control the gut inflammation by taking probiotics, that will allow the leaky gut wall to heal. They found the following bacteria to be beneficial:

  • Lactobacillus casei
  • Lactobacillus rhamnosus
  • Streptococcus thermophilus
  • Bifidobacterium breve
  • Lactobacillus acidophilus
  • Bifidobacterium longum
  • Lactobacillus bulgaricus

Probiotics usually contain most or all of these good bacteria. This allows the inflamed gut wall to heal and restore the gut barrier.

Two human trials

A  randomized, double-blind, placebo-controlled clinical trial was published in 2014. 52 patients with NAFLD who were overweight or obese were taking probiotics twice a day versus a placebo control group. After 28 weeks the experimental group showed normalized liver enzymes and a significantly reduced C-reactive protein. The C-reactive protein was reduced by 54% in comparison to the placebo group.

The other clinical trial consisted of 50 patients with NAFLD, but a normal or low body mass index. Half of the patients were taking probiotics, the other half placebo pills. The researchers noted that after 28 weeks the inflammatory markers were much lower in the experimental group than in the placebo group. Also, the liver values were now normal.

Discussion

Patients with non-alcoholic fatty liver disease are in danger of progressing to liver cirrhosis and liver failure. At this point only a liver transplant could rescue them from death. However, as the clinical trials have shown, the use of probiotics to normalize the gut bacteria can normalize the inflammation of the gut and reverse fatty liver disease. Regular exercise and weight loss through the use of a lower calorie diet will also help in normalizing the liver function. When prebiotics are added to probiotics the healing effect is amplified even further. Prebiotics contain fiber that cannot be digested, but which is useful for bacteria from the probiotics to multiply on.

We need to pay more attention to the food we are consuming. For instance, regular beef in the US contains antibiotic residues that permanently change the gut flora. This leads to an inflammatory reaction of the gut wall and leaky gut syndrome. Gut bacteria and toxic substances can now enter into the portal vein and reach the liver. The liver attempts to detoxify this, but in the process, it also develops non-alcoholic fatty liver disease (NAFLD). About 25% of Americans have this condition, which is asymptomatic. The only way to diagnose it is through blood tests and scans. Two clinical trials have documented that NAFLD can reverse back to normal with the consumption of probiotics. Regular exercise and weight loss (calorie-reduced diet) also help to reverse NAFLD back to normal liver tissue. It is obvious that a timely intervention is necessary to avoid the progression to liver cirrhosis!

How Healthy Is Food From The Supermarket?

I am asking the question here: “how healthy is food from the supermarket?” We tend to assume that food we buy at the supermarket would be safe to eat. Think again, because below I will explain why it is NOT safe.

The “dirty dozen”

You may or may not have heard about the “dirty dozen”. The Environmental Working Group (EWG) is a non-profit organization that monitors fruit and vegetables for insecticide residues. For 2019 they have determined these unhealthy dozen of fruit and vegetables: strawberries, spinach, kale, nectarines, apples, grapes, peaches, cherries, pears, tomatoes, celery, potatoes and peppers (hot and bell peppers). Yes, you counted right, this “dozen” actually consists of 13 items.

When I want to buy any of these fruit, I strictly get fruit and vegetables, which are organic. If this is not available in the regular supermarket, I go to the health food store and buy organic fruit and vegetables there. Local farmers’ markets also offer organically grown produce. Every year the EWG brings out an up-to-date list of the dirty dozen. If an item is not on this list, you can assume that it is safe to eat.

Sugar and too many starches are the next problem

How healthy is food from the supermarket? We said we wanted healthy food. Unfortunately ample consumption of sugar and starchy foods end up as belly fat and also as atheromatous deposits in the arteries. I do not eat bread unless it is 100% rye, but then only 1 or 2 slices occasionally. I do not eat potatoes, rice or pasta. Those who love pasta can eat the pasta-look-alike, called shirataki, which is made from the glucomannan root (also known as konjac root). The carbs here are bound to fiber, which digest a lot slower than regular pasta. So, this means that you are bypassing the section in the supermarket where they sell muffins, donuts etc.

Processed foods

In the center of the supermarket you find all kinds of processed foods. They are usually full of sugar, salt and unhealthy fats (hydrogenated fats). People who eat a lot of processed foods are more prone to get heart attacks, strokes and may even come down with cancer. It is not the kind of food that I want. Instead I like whole foods like organic apples, berries and raw or cooked vegetables.

Fish section

I like salmon, sole, cod, halibut, trout, mussels, shrimp and squid. But I am careful not to load up on the big predator fish like tuna. One has to be careful about methylmercury content in fish. I reviewed fish, mercury contamination and the benefits in this link.

As this link shows, there are low, medium and high methylmercury contaminated fish. The higher the fish is in the predator chain, the higher the methylmercury content. This is of particular concern for pregnant women as mercury is toxic for the fetal brain tissue. But they can consume low mercury fish and shellfish, like mackerel, herring, wild salmon, shrimps or clams. I figure, what’s good for women who are pregnant is good for other people. As a result, I limit my fish meals to the low methylmercury contaminated fish category.

How healthy is food from the supermarket – Meat section

Now your shopping trip becomes problematical. There has been news from the Word Health Organization at the end of 2015 that red meat and red meat products are class 1 carcinogens.

This would mean that you may come down with colorectal cancer, if you consume red meat and red meat products regularly. This is when I stopped eating red meat and red meat products like prosciutto. Unfortunately this warning includes pork, lamb and all sausages as well. In the meantime further research has shown that antibiotics that are fed to beef before they are slaughtered get into the meat and change the human bowel flora when red meat or red meat products are consumed.

Trimethylamine N-oxide (TMAO)

There is a change in bowel flora that causes chemical reactions in the gut with the consumption of beef and eggs. Beef contains carnitine, which can lead to the production of TMAO (trimethylamine N-oxide).

Egg yolk contains choline, which also raises TMAO levels in the blood.

In the following study 113 healthy men and women consumed either a meat diet (beef), white meat diet or protein from non-meat sources. After one month the beef group had triple the amount of TMAO in their blood compared to the other two diets.

Interestingly, when the diets were switched the TMAO levels normalized again in the former beefeaters, when consuming white meat or protein from non-meat sources.

What meat am I shopping for?

I recommend not to eat beef more often than once per week and to eat only grass-fed beef. I eat organic chicken and ground turkey breast meat from health food stores (the turkeys are NOT fed antibiotics). Otherwise I eat seafood, cheese from non-recombinant growth hormone milk. This means that I only buy European import cheese or Canadian cheese. In the US you have to ask for organic milk and organic milk products.

The deep frozen section

I look for deep frozen vegetables, fruit, and fish as well as meats. As vegetables are quickly readied for the freezer, their vitamin content can be higher than that of a vegetable that has spent 8 days in transit from the field to the produce department. The deep frozen section also gives you access to a lot of variety. You’ll be able to enjoy some organic strawberries, even when they are not in season. Read the labels, as some fruit have been packaged with sugar syrup. Look for the varieties, where no sugar has been added. The frozen section also contains some highly processed items: deep-fried foods and dessert selections, which have nothing to do with health, so I avoid them.

Canned foods

Canned foods can be useful, as long as you are dealing with fruit that are canned in their juices and not in sugar syrup. The vegetables are less valuable in vitamins than their deep frozen counterparts. Watch out for varieties, where less salt is added. The label will tell you ”low sodium”. Also pay attention to the cans. Those that are not lined with BPH have a label that emphasizes this. You can avoid this by buying canned food in glass jars, such as tomato sauce.

Cosmetics

You will not have to navigate all the aisles, except for your cleaning products and your cosmetics. These have their own problems: lots of cosmetics contain Parabens. Leave them on the shelf, and be aware that some cleaning products can be hazardous too. Pick environmentally friendly, non-toxic products!

Staples you need

There are some staples, which you will also require: olive oil, some olives, almonds or macadamia nuts (preferably raw). The one cereal product, which is valuable will be coarse rolled oats and some pot barley. Both varieties carry a lot of fibre, which makes them very useful food staples. Avoid the “quick cooking” or “instant” oats. Due to the processing, the carbohydrates are absorbed a lot faster and consequently trigger a higher insulin response.

Drinks

You will wonder about drinks next. Having passed the colas, ginger ales and other sugar pops you may eye the diet drinks. Beware of drinks sweetened with aspartame. There is increasing evidence that phenylalanine (brand names: Aspartame, NutraSweet and Sweet’N Low) is not a “harmless” sweetener. Newer research has shown that it can cause gastroesophageal reflux (=GERD) and migraine headaches. Stevia, a sweetener from a South American plant, does not have harmful effects. Stevia is safe to use, as it does not cause an insulin response. You are best served with mineral water, purified drinking water, herb teas, tea or coffee. Fruit juices do have vitamins and minerals, but they are high in sugar causing an insulin release.

What is in fruit juices?

You would not really eat 3 large apples in one sitting? Probably not! So why insist on drinking 8 oz. of apple juice? You’ll ingest all the sugar and forgo the fibre! You also notice that a lot of fruit juices have been mixed with sugar, water, artificial flavor, some color, and as an apology some vitamin C has been added. They are appearing on the shelves as “a good source of vitamin C”. In reality we are dealing with flavoured, colorful sugar water. Use your own judgement, whether you want to spend your dollars on this selection!

Power bars, snacks

In the aisle adjacent to the pop you will very likely encounter a huge selection of convenience and snack foods. They have several things in common: you have met them on TV, some will be high in starches and fat (chips), others will be high in starches, sugar, and fat (cookies, donuts, cream pastries), and we are dealing with trans fats. Do take time to read the listed ingredients, and then decide, whether you and those who eat in your household deserve nutritional garbage.

We have now completed our round trip in the supermarket. You may still be in mild shock, noticing that “healthy” foods are not the general rule on the shelves. When we buy vegetables and fruit we have to be aware of the “dirty dozen”, which is contaminated with insecticides. We will not buy anything on that list, but buy instead the organic version. You may have to go to the health food store to get these items. Red meat and red meat products produce carcinogens in the human gut. It seems to stem from antibiotics that are used in beef farms and chicken farms. With chicken I buy organic chicken. With beef you need to be more careful and reduce the consumption to once per week and eat only grass-fed beef.

It is tricky to buy healthy food from the supermarket

But I have added many other comments to it.

Asparagus contains glutathione, a powerful antioxidant that neutralizes free radicals and carcinogens. It is said to help prevent lung cancers, larynx cancer, and bone, breast and colon cancers.

Berries like strawberries, raspberries, blackberries and blueberries are full of bioflavonoids. These are antioxidants, which prevent cardiovascular disease.

Note that flaxseeds must be ground to powder to release the nutrients from its tough shell. Both chia and flaxseeds can be used as an egg replacement in vegan cooking.

Fish high in omega-3s

Omega-3 fatty acids are abundantly present in fish. It has plaque-reducing properties and also reduces the risk for abnormal heart beats. Overall this means less cardiovascular disease. The American Heart Association recommends a 3.5 oz. serving of fatty fish (salmon, mackerel, lake trout, herring or sardines) twice per week.

Coffee

Green tea is known to reduce blood pressure slightly and to reduce cholesterol. Both effects are beneficial for the cardiovascular system (prevents heart attacks and strokes). Green tea also prevents many cancers. Whatever we know about coffee consumption seems to also be true for green tea consumption.

Nuts

Nuts contain healthy fatty acids (omega-3). But they also contain fiber, protein, minerals, vitamins and antioxidants. Use them in desserts, in salads and as a quick food on the go. They are definitely healthier than protein bars.

Liver

Liver is one of the nutrient rich foods. It is rich in iron, phosphorus, vitamin A, folate, vitamin B12 and biotin.

Oatmeal

Oatmeal is rich in soluble fiber. It has been shown to lower LDL cholesterol and total cholesterol. This is important for prevention of heart disease.

Red wine (may be)

The bioflavonoids of red grapes have been the subject of much research. There is a dose-response curve showing a protective effect with regard to heart attacks and strokes with the consumption of  1 to 3 glasses of red wine per day. But unfortunately there is also a dose-response curve with respect to alcohol consumption and cancer causation. Personally, I take resveratrol from the health food store, 500 mg daily and consume white wine or red wine very rarely.

Tomatoes

There are a number of beneficial phytochemicals in tomatoes. Carotenoids like lycopene, lutein, zeaxanthine and beta-carotenes are helping to prevent prostate cancer and colon cancer. Potassium and folate are cardioprotective.

Spinach

Eating spinach regularly will provide you with magnesium, iron and bioflavonoids. Magnesium is good for a regular heart rhythm. The other nutrients are good for skin, bone and hair health.

Discussion

I have reviewed why these 16 foods are the best foods for your heart. We have seen that many foods that are rich in antioxidants are also cancer preventative. People who eat a Mediterranean diet will get these 16 foods, because their meals are balanced with nutrients. But if you eat a hamburger or a pizza you will not get balanced nutrients. The more one-sided your food intake, the more dangerous your lifestyle becomes. This is the problem with the Standard American diet (“SAD”). You need all of the components of the 16 foods described here. Junk food won’t do, as it consists only of empty calories.

Combine nut supplements and adopt a healthy lifestyle

This said, you know now about the results of good research. It is best to not be concerned about biases any more. For the average consumer it is important to apply what we know: use nuts regularly (about 30 grams per day, which is a handful). But it would not be a bad idea to adopt some of the other healthy lifestyles that nut eaters tend to have. In other words, exercise regularly, don’t smoke, cut out sugar, drink less alcohol and eat fruit and vegetables regularly. This way you will amplify the health promoting effect of the nut supplements.

Studies have shown that the body is able to handle these natural trans fats and heart attacks are not more frequent in people eating moderate amounts of these products including butter from cows that graze on pasture.

Artificial trans fats

Quite the opposite is true for artificial trans fats in margarine that comes from vegetable oil. Avoid bakery items like sweet pieces or muffins and other products that contain hydrogenated oils. Read labels! Use olive oil or coconut oil, but avoid vegetable oils like corn oil, safflower oil or grape seed oil to get away from trans fats and unstable oils that turn rancid. Rancid oils contain free radicals that oxidize LDL cholesterol and attack the lining of your arteries.

3. Cut out sugar and starchy foods

You need to cut out sugar and starchy foods because these will raise your LDL cholesterol and triglycerides, which also leads to hardening of your arteries. This is an important observation. Starchy foods are broken down by  pancreatic juices into sugar, which enters your blood stream, causing an outpouring of insulin from the pancreas. When the short-term storage of sugar as glycogen is exhausted in muscle and liver tissue, the liver has to process any surplus of sugar that is still there. The end results are triglycerides and LDL cholesterol. Unfortunately the protective HDL cholesterol does not reach higher levels, when the LDL cholesterol is increased. A persistent diet of high-refined carbs will increase the risk for heart attacks and strokes. It follows from this that we are all better off cutting out sugar and starchy foods from our food intake.

4. Increase your soluble fiber intake

Increase your soluble fiber intake by eating vegetables, oats and oat bran, lentils, fruits and beans. Why does this decrease LDL cholesterol? The liver tries to eliminate too much cholesterol by binding it to bile salts and excreting it into your small bowel. But the last part of the small bowel reabsorbs some of these bile salts , and from there they return to the liver. This is called the enterohepatic pathway of bile salts. Soluble fiber intake binds those bile salts and prevents re-absorption in the enterohepatic pathway, eliminating cholesterol safely in stool. Soluble fiber from psyllium, pectin, beta-glucans and others have been shown in clinical trials to reduce LDL cholesterol by binding bile salts in the gut (interrupting the enterohepatic pathway).

Plant sterols and fiber supplements

Plant sterols (usually sold as sterol esters) are recognized by the FDA as reducing the risk of coronary heart disease, if taken in high enough amounts (2.4 grams of sterol esters per day). There are other useful supplements like artichoke extract, pomegranate, soy protein, Indian gooseberry (Amla), garlic and pantethine (vitamin B5) that are beneficial in terms of prevention of heart attacks and strokes. It would be too lengthy to get into more details here.

5. Take a whey protein supplement

There are two major milk proteins, whey and casein. Only whey protein binds to total and LDL cholesterol, lowering both. It is available in health food stores. Follow the package insert of the whey product for dosing.

6. Increase your omega-3 fatty acid intake

Omega-3 fats are healthy fats naturally present in fish oils and nuts. They increase the amount of circulating HDL cholesterol, which binds the bad LDL cholesterol. Go ahead and eat salmon, herring and mackerel as well as walnuts, ground flaxseeds and almonds. You can also take molecularly distilled (or pharmaceutically pure) EPA/DHA supplements. This pure form of fish oil is free of mercury and other heavy metals. EPA stands for eicosapentaenoic acid or omega-3 fatty acid. DHA is the acronym for docosahexaenoic acid, an important supplement for the brain. Tests have shown that fish oil supplements at a dosage of 3.35 grams per day of EPA plus DHA reduce triglycerides by up to 40%, equally to Lipitor or even more effective, but without the statin side effects. The end result is that your total cholesterol/HDL ratio decreases. This reduces the risk for heart attacks and strokes.

7. Eat foods with anthocyanin

In a 24-week study with diabetic people HDL levels rose by 19% when food was eaten that was rich in anthocyanin. This consisted of eggplant, purple corn, red cabbage, blueberries and blackberries. The advantage of raising the HDL cholesterol level is that the total cholesterol to HDL ratio decreases, which lowers the risk for heart attacks and strokes.

8. Exercising regularly

Exercising will increase your HDL cholesterol, which again decreases the ratio of total cholesterol to HDL cholesterol. This number should be between 1 and 3.5, the lower, the better.

9. Take a supplement called Ubiquinol, or Co-Q-10

Adults above the age of 60 need 400 mg once daily, younger people need between 200 mg and 300 mg daily. Co-Q-10 prevents oxidation of LDL cholesterol, which would aggressively attack the arterial walls causing hardening of the arteries. What causes oxidation of cholesterol? The answer is clear: fried foods like french fries or deep fried chicken will lead to oxidation; other culprits are margarine, commercially baked goods and cigarette smoking.

10. Calcium and vitamin D3

Recently a study on postmenopausal and overweight or obese women found that supplements of calcium combined with vitamin D3 lowered cholesterol.

11. Polyphenols

Flavonoids are the largest group among the polyphenols  in such common foods as vegetables, fruits, tea, coffee, chocolate and wine. Over 130 studies on humans have shown improvement of the lining of the arteries (endothelial functioning) and lowering of blood pressure. Polyphenol consumption has a connection  to a lower risk of mortality from heart attacks. Eat a Mediterranean type diet or a DASH diet, and you will automatically get enough polyphenols with your food. However, resveratrol, the powerful red wine polyphenol, warrants a separate daily supplementation as it prevents LDL oxidation in humans (Ref.1). Take about 250 mg of resveratrol daily.

12. Niacin/ nicotinic acid

This supplement comes as “flush-free niacin” and also as extended release niacin; it can raise the beneficial HDL cholesterol by 30 to 35% when patients take higher doses of 2.25 grams per day. In a metaanalysis of 7 studies researchers found a significant reduction of heart attacks and transient ischemic attacks (precursor syndrome before developing a stroke). Niacin can change the small particle LDL into a large particle size LDL, which is less dangerous. Niacin also reduces oxidation of LDL, which stops the atherosclerotic process. For a healthy person 500 mg per day of flush-free niacin is adequate.

13. Curcumin

This is a powerful heart and brain protector combining three different mechanisms in one; it is reducing oxidative stress, is an anti-inflammatory and counters the process that threatens to destroy the lining of the arteries. One study on healthy volunteers showed reduction of 33% in lipid oxidation, a 12% reduction of total cholesterol and an increase of 29% of the protective HDL cholesterol when patients took 500 mg of curcumin for only 7 days (Ref.1). This is the daily dose I would recommend for prevention of heart attacks and strokes.

14. Vitamin E (tocopherols)

This fat soluble vitamin is an antioxidant and in the past knew about its use as being heart supportive. Strangely enough some conservative physicians bad-mouthed this vitamin. In the meantime health practitioners have returned to using the vitamin. It turns out that there are 8 different types of tocopherols, with the alpha tocopherol being the best-known, but you also want to be sure that you are getting gamma tocopherol with your balanced vitamin E supplement every day. It remains the one that is a powerful anti-inflammatory. Simply ask staff at your health food store for a vitamin E supplement with gamma tocopherol in it. Take 400 IU per day (of the mix).

Conclusion

There is a lot you can do to control your cholesterol level by changing your diet, shedding some pounds, exercising and quitting smoking. All this will contribute to lower cholesterol. This will have a beneficial, long-term effect regarding prevention of heart attacks and strokes. In addition there are specific supplements and vitamins, which prevent heart attacks and strokes as well. With these measures the majority of people with high LDL cholesterol can change their cholesterol levels without taking statins. Statins have serious side effects like Alzheimer’s disease and a painful muscle condition called rhabdomyolysis. None of the other measures described here have any such side effect. Even if you chose to only concentrate on a few of these 14 points to lower cholesterol there will be significant improvements in your LDL and HDL cholesterol levels, when you ask your doctor to order these blood tests.

Breast Cancer Risks

Dr. David Zava, PhD gave a talk on breast cancer risks. His presentation took place at the 24th Annual World Congress on Anti-Aging Medicine (Dec. 9-11, 2016) in Las Vegas that I attended. The detailed title was: “The Role of Hormones, Essential Nutrients, Environmental Toxins, and Lifestyle Choices on Breast Cancer Risk”.

He pointed out that both estrogens and progesterone are safe hormones, as long as the doctor does not overdose them and keeps a hormone balance. Unfortunately many women in menopause have too much estrogen on board as the ovaries are still producing them, but there is a lack of progesterone, the moderating hormone that makes estrogen safe.

In the following I am summarizing Dr. Zava’s talk with regard to the essential messages, but leave away much of the highly technical detail of the presentation. This would dilute the message of this blog. I will include a few links for those who wish to read more details about the topic.

Balance between estrogen and progesterone

Most of her life a woman is used to cyclical hormone changes between estrogen and progesterone. When a woman no longer ovulates in premenopause and menopause there is a surplus of estrogen and a lack of progesterone. Having no ovulation means that there is no corpus luteum developing, which is where in the past progesterone production took place. This creates a disbalance where estrogen is dominating; it is called “estrogen dominance”.

This is a dangerous hormone disbalance, because the breast ducts experience a growth stimulus, but the modifying, calming effect of progesterone is missing. Mixed into this is that the stress hormone, cortisol also can make the effect of estrogen worse. On the other hand Dr. Zava showed slides from studies documenting replacement of missing progesterone with a skin progesterone cream (percutaneous bioidentical progesterone cream).

Progesterone concentration in breast lumps after progesterone cream applications

Plasma and breast tissue concentration of progesterone were measured in 40 premenstrual women. The diagnoses were breast lumps and the physicians arranged surgery for them. One group received progesterone cream treatment for 10 to 13 days; the other group was the placebo group. At the time of surgery the plasma (blood) values of progesterone were the same, but progesterone levels in breast tissue were more than 100-fold higher than the values from the placebo group who had received a neutral skin cream. The same experiment also showed that progesterone reduced the number of proliferating epithelial cells (experimental progesterone group). Estrogen on the other hand led to an increase of the number of proliferating epithelial cells (placebo group).

Progesterone cream applied to breasts of premenopausal women

Another example that Dr. Zava gave was a study where 25 mg of bioidentical progesterone cream applied directly to breasts of premenopausal women increased breast tissue progesterone 100-fold, while blood concentrations of progesterone remained the same. Again progesterone decreased the breast stimulation by estrogen of normal epithelium cells.

How to measure progesterone levels

Dr. Zava who runs the ZRT laboratory spent some time to explain how to measure progesterone in a physiological way. He said that these experiments and others that he also projected tell a clear story. Blood (serum) progesterone levels do not adequately reflect what tissue levels in a woman’s breasts are. On the other hand saliva hormone levels do give an accurate account of what breast tissue levels are like. A woman received 30 mg of topical progesterone application. She then had hourly progesterone levels in the serum and in the saliva done. The serum progesterone levels remained at around 2 ng/ml, while the saliva progesterone levels peaked 3 to 5 hours after the application. It reached 16 ng/ml in saliva, which also represents the breast tissue progesterone level.

Blood progesterone levels are unreliable

As a result, Dr. Zava said that the important lesson to learn from this is not to trust blood progesterone levels. Too many physicians fall into this trap and order too much progesterone cream, which leads to overdosing progesterone. In contrast, with salivary progesterone levels you see the physiological tissue levels, with blood tests you don’t. Dr. Zava said: avoid using venipuncture blood or urine in an attempt to interpret hormone test levels, as you will underestimate bio-potency and overdose the patient.

Historical failure of estrogen replacement therapy (ERT)

A review of breast cancer would not be complete without mentioning the Women’s Health Initiative (WHI). The U.S. National Institutes of Health (NIH) initiated this trial in 1991.

Researchers prematurely terminated Women’s Health initiative

The WHI ended suddenly in July 2002. The authors stated: “The overall health risks exceeded benefits from use of combined estrogen plus progestin for an average 5.2 year follow-up among healthy postmenopausal US women.” The study found a 41% increase in strokes, 29% increase in heart attacks, 26% increase in breast cancer, 22% increase in total cardiovascular disease, a doubling of blood clots. The recommendation made by this study was to discontinue PremPro.

Breast cancer in the Million Women Study from synthetic hormones

Another study that was mentioned was “Breast cancer and hormone-replacement therapy in the Million Women Study”.  In this study postmenopausal women received HRT with synthetic hormones, either estrogen alone or estrogen mixed with a progestin (in British English “progestagen”). After 5 years estrogen alone had a 30% increased risk of developing breast cancer. HRT with an estrogen-progestagen mix had a 100% increased risk of developing breast cancer.

Huge difference between bioidentical hormones and synthetic hormones

Unfortunately in both of these human experiments the researchers used the wrong hormone substances, namely synthetic estrogens and synthetic progestins. They are NOT identical with natural estrogens and progesterone that a woman’s body makes. As long as the hormones used for hormone replacement therapy are chemically identical to the natural hormones, the body will accept them as they fit the natural hormone receptors in the body. It is the misfit of synthetic hormones that blocks the estrogen receptors or the progesterone receptors. You can readily see from the illustrations of this link that there is a fine balance between the workings of these receptors and there is absolutely no room for patented side chains that Big Pharma introduced into synthetic HRT hormones.

Individualizing bioidentical hormone prescriptions based on blood tests

The other problem of both these studies was that every woman was getting the same dose of hormones and that nobody measured their estrogen blood or estrogen saliva hormone levels. In retrospect the regulatory agencies should never have allowed these “hormones” to hit the market.

Breast cancer develops in three stages

Dr. Zava explained that it common knowledge for some time that breast cancer develops by going through 3 stages.

  1. Initiation

First of all, damage to the DNA of one of the cells types in the breast is what starts the process in the development of breast cancer. This can be done by catechol estrogen-3,4-quinones as was shown by these researchers.

Aromatase inhibitors is useful to reduce estrogen in overweight or obese women where aromatase is present in fatty tissue. The reason obese women have more breast cancer is likely from the extra estrogen production from androgens. Aromatase converts these male hormones from the adrenal glands into estrogen.

Iodine/iodide alters gene expression, which reduces breast cancer development, but also slows down cell division in existing breast cancer. The authors suggested to use iodine/iodide supplements as adjuvant therapy in breast cancer treatment.

  1. Promotion

Furthermore, the next step is that something has to promote the DNA mutation into becoming part of a cancer cell. Estrogen quinones are dangerous estrogen metabolites. They can form from catechol estrogens (other metabolites of estrogen) by reactive oxygen species. But selenium, a trace mineral can interrupt the formation of estrogen quinones, which stops the breasts cancer promotion process. A study from the Klang Valley, Malaysia showed that selenium showed a dose-response effect with respect to prevention of breast cancer; the more selenium in the food, the less breast cancer occurred.

  1. Progression (includes invasion and metastases)

Finally, several factors can help the breast cancer cells to progress, grow bigger locally and eventually move into other areas of the body as metastases. Dr. Zava showed several slides where details of metabolic processes were shown and how changes in some of these would lead to progression of breast cancer. Estrogen excess is a common pathway to breast cancer. The key is to balance it with progesterone, supplements, remove anything that causes estrogen overproduction like obesity (via the aromatase pathway).

The fallacy of overdosing or underdosing

When estrogen is overdosed, it becomes aggressive as indicated before; it can initiate DNA mutations that can cause breast cancer. If it is under dosed, the lack of estrogen can cause heart attacks, strokes and osteoporosis. When estrogen is balanced with progesterone a postmenopausal woman feels best and she is protected from the negative effects of estrogen.

Measures that help prevent breast cancer

Supplement only with bioidentical hormones

When supplementing with bioidentical hormones, keep estrogen within physiological limits and don’t overdose. This can be measured through blood tests or saliva hormone tests. Your most important natural opponent of estrogen is progesterone, which is usually missing in menopause. Measure hormones using tests (progesterone only with saliva tests, estrogen either by blood tests or saliva tests). Don’t rely going by symptoms.

Progesterone to estrogen ratio

Keep the progesterone to estrogen ratio (Pg/E2) at an optimal range, which is in the 100- to 500-fold range. Measure the saliva hormone level of both progesterone and estrogen and calculate. Remember that progesterone serum levels are meaningless. The much higher progesterone level protects the postmenopausal woman from estrogen side effects. Here is a statement worth noting: “Until evidence is found to the contrary, bioidentical hormones remain the preferred method of HRT.” This was the conclusion of a study using bioidentical hormones, where the protection from breast cancer and heart attacks and strokes was also noted.

Eat more fiber containing foods and less beef

Increase fiber intake and reduce red meat consumption. This will eliminate conjugated steroid hormones in the stool. It also increases the sex hormone binding globulin in the blood, which limits the bioavailability of estrogens. Fiber absorbs bile toxins and removes them from the body.

Calcium supplement

Calcium-D-glucarate is a supplement that will decrease beta-glucuronidase. The estrogens were conjugated with the purpose to be eliminated, but beta-glucuronidase causes the conjugated estrogens to be reabsorbed.

Reduce breast cancer risk with probiotics

Probiotics likely stimulate the immune system and help reduce the risk of breast cancer.

No pollutants and toxic chemicals

Avoid toxins like petrochemical pollutants and toxic chemicals. Avoid trans fats. If toxic, heavy metals are present (arsenic, cadmium, lead, mercury) remove these. Some naturopaths use EDTA chelation to do this.

Other useful supplements

Supplements: sulforaphane (broccoli), EGCG (green tea), alpha-lipoic acid (antioxidant), cruciferous vegetables, resveratrol, selenium and iodide/iodine, N-acetyl cysteine-glutathione. All these supplements/nutrients will prevent estrogen to go to the “dark side”. The dark side is the formation of toxic 4-OH estrogen that could further be converted into catechol estrogen-3,4-quinones that can damage DNA and cause mutations.

Methylation of catechol estrogens

Increase methylation of catechol estrogens: vitamin B1, B6, B12 and folic acid. Methyl donors also are useful for this purpose: MSM (methylsulfonylmethane), SAMe, and Betaine.

Healthy lifestyle (diet , exercise) helps your immune system

Improve your diet (Mediterranean type), exercise moderately, reduce stress, and replace hormones in physiological doses as discussed under point 1 and 2.

Estrogens, when unopposed by enough progesterone, can cause mutations in breast tissue of women and cause breast cancer. He also reviewed two major clinical trials that utilized hormone replacement therapy (HRT). The problems with these were the synthetic estrogen hormones that caused breast cancer and the synthetic progestins that also behaved like estrogens (not like progesterone) and caused even more breast cancer. The lesson from this is that only bioidentical estrogens and progesterone work in hormone replacement for menopause. Also, the hormones balance each other as discussed under measures that help to prevent breast cancer. In addition there was a list of other useful supplements given that can be taken to reduce the danger of breast cancer.
