Sleepless Nights

Sleeping problems (insomnia) are very common. About 10% of the population suffers from chronic insomnia; 30% of the population suffers from occasional sleep problems. In a large outpatient population of a clinic consisting of 3500 patients who had at least one major clinical condition, 50% complained of insomnia, 16% had severe symptoms, 34% had mild symptoms (Ref.1). Insomnia is more common among women, and older people as well as in people with medical or psychiatric illnesses. Long-term studies have shown that the same insomnia problems persist throughout many years. It is not possible to offer a simple remedy for insomnia, because insomnia is a complex problem. Here I will discuss some of the causes of insomnia and also discuss some of the treatment options.

Symptoms of insomnia

The person who suffers from insomnia will usually state that they have problems falling asleep. Worries of the day suddenly circulate through their thoughts and they toss and turn nervously looking at the clock from time to time and getting more and more anxious that they cannot sleep. Others fall asleep OK, but in the middle of the night they wake up perhaps to visit the restroom, but then they cannot go back to sleep. Others wake up 2 hours before their normal alarm clock time and they feel their stomach rumbling making it impossible to fall back to sleep. Older people with chronic diseases and general poor health suffer more from insomnia. In this setting insomnia may be more related to the underlying disease rather than old age. Psychiatric disorders also are associated with more insomnia. Treat the underlying psychiatric illness, and the insomnia disappears.

Although insomnia is a sleep disturbance during the nighttime, people who are affected with this complain of daytime fatigue, of overstimulation, yet they catch themselves making frequent mistakes, and their inability to pay attention gets them involved in accidents and falls. Longitudinal studies have shown (Ref. 1) that people with chronic insomnia are more likely to develop psychiatric disease, such as major depression,  anxiety disorder and alcohol and substance abuse. Unfortunately these disorders can by themselves again cause insomnia, which reinforces chronic insomnia. Insomnia leads to poorer social and physical functioning, affects emotions, leads to a lack of vitality and physical endurance, contributes to worsening of pain and can affect general and mental health.

Research about insomnia

Much has been learnt from sleep studies using polysomnography monitoring during a full night’s sleep. These studies have been used mainly as a research tool. In such studies eye movements, brain wave activity, muscle activity, chest movements, airflow, heart beats, oxygen saturation and snoring (with a microphone) are all simultaneously recorded. This way restless leg syndrome, sleep apnea, snoring, seizure disorders, deep depression etc. that can all lead to insomnia can be diagnosed and separated from insomnia. The stages of sleep (wakefulness, stage 1 to 3 sleep and the REM sleep stage) can also be readily measured using polysomnography (Ref.2). According to this reference the majority of insomnia cases do not need this complex procedure done.

Causes of insomnia

Traditionally insomnia cases are classified into primary insomnia and secondary insomnia. Secondary insomnia is caused by all of the factors discussed below. When they are dealt with, we are left with cases of primary insomnia.

The following medical conditions can cause insomnia: heart disease, pulmonary diseases like asthma and chronic obstructive pulmonary disease (COPD); gastrointestinal disease like liver cirrhosis, pancreatitis, irritable bowel syndrome, ulcers, colitis, Crohn’s disease; chronic kidney disease; musculoskeletal disease like arthritis, fractures, osteoporosis; neurodegenerative disease like MS, Parkinson’s disease, Alzheimer’s disease; endocrine disease like diabetes, hyper- or hypothyroidism, adrenal gland fatigue and insufficiency; and chronic pain conditions. Also, psychiatric conditions like major depression, schizophrenia and anxiety disorders can cause insomnia.

This list in not complete, but it gives you an idea of how complex the topic of insomnia is.
The physician who is seeing a patient with insomnia needs to rule out any of these other causes of insomnia to be certain that the only condition that is left to treat in the patient is insomnia itself. The other diagnoses have to be dealt with separately or else treatment of insomnia will fail.

Ref. 1 points to a useful model of how to think about causation of insomnia: there are three points to consider, namely predisposing, precipitating, and perpetuating factors. Let’s briefly discuss some of these.

Predisposing factors

We are all different in our personal make-up. If you are well grounded, chances are you are not susceptible to insomnia. Anxious persons or persons who have been through a lot of negative experiences in life will have personality traits that make them more prone to insomnia. Lifestyle choices such as late nights out, drinking with the buddies in a bar (extreme circadian phase tendencies) will have an impact on whether or not you develop insomnia.

Precipitating factors

A situational crisis like a job change or the death of a loved one can initiate insomnia.  However, there could be a medical illness such as a heart attack, a stroke or the new diagnosis of a psychiatric illness that has become a precipitating factor. Sleep apnea and restless leg syndrome belong into this group as well as would the stimulating effect of coffee and caffeine containing drinks. Jet lag and nighttime shift work can also be precipitating factors.

Perpetuating factors

Daytime napping to make up for lost sleep the night before can undermine sleep initiation the following night, which can lead to a vicious cycle. Similarly, the use of bedtime alcoholic drinks leads to sleep disruption later that night and can become a perpetuating factor, if this habit is maintained. Even the psychological conditioning of being anxious about whether or not you will fall asleep easily or not the next night can become a perpetuating factor.

I will return to this classification and the factor model of causation of insomnia when we address treatment options.

Drugs that can cause insomnia

One major possible cause for insomnia  can be side effects from medications that patients are on (would belong to the ‘perpetuating factors’ among causes). Physicians call this “iatrogenic insomnia”. The antidepressants, called selective serotonin reuptake inhibitors (SSRI’s) like Prozac are particularly troublesome with regard to causing insomnia as a side effect. Other antidepressants like trazodone (Desyrel) are used in small doses to help patients with insomnia to fall asleep. Some asthmatics and people with autoimmune diseases may be on prednisone, a corticosteroid drug. This can cause insomnia, particularly in higher doses; so can decongestants you may use for allergies; beta-blockers used for heart disease and hypertension treatment; theophylline, an asthma medication and diuretics. Central nervous stimulants like caffeine or illicit drugs can also cause insomnia. Hormone disbalance in general and hyperthyroidism specifically as well as Cushing’s disease, where cortisol levels are high will cause insomnia.

Treatment of insomnia

So, how should the physician approach a patient with insomnia? First it has to be established whether there is secondary insomnia present due to one of the predisposing, precipitating or perpetuating factors. In other words, is there secondary insomnia due to other underlying illnesses? If so, these are being addressed first. Lifestyle choices (staying up late every night) would have to be changed; alcohol and drug abuse and overindulging in coffee or caffeine containing drinks needs to be dealt with. Cognitive therapy may be beneficial when mild depression or anxiety is a contributing factor to insomnia.

The remaining insomnia (also medically termed “primary insomnia”) is now being treated.

The following general points are useful to get into the sleeping mode (modified from Ref. 3):

  1. Ensure your bedroom is dark, soundproof, and comfortable with the room temperature being not too warm, and you develop a “sleep hygiene”. This means you get to sleep around the same time each night, have some down time 1 hour or so before going to bed and get up after your average fill of sleep (for most people between 7 to 9 hours). Do not sleep in, but use an alarm clock to help you get into your sleep routine.
  2. Avoid caffeine drinks, alcohol, nicotine and recreational drugs. If you must smoke, don’t smoke later than 7PM.
  3. Get into a regular exercise program, either at home or at a gym.
  4. Avoid a heavy meal late at night. A light snack including some warm milk would be OK.
  5. Do not use your bedroom as an office, reading place or media center. This would condition you to be awake.  Reserve your bedroom use only for intimacy and sleeping.
  6. If you wake up at night and you are wide awake, leave the bedroom and sit in the living room doing something until you feel tired and then return to bed.
  7. A self-hypnosis recording is a useful adjunct to a sleep routine. Listen to it when you go to bed to give you something to focus on (low volume) and you will find it easier to stop thinking.

Drugs and supplements for insomnia

1. In the past benzodiazepines, such as diazepam (Valium), lorazepam (Ativan), fluorazepam (Dalmane), temazepam (Restoril), triazolam (Halcion) and others were and still are used as sleeping pills. However, it was noted that there are significant side effects with this group of drugs. Notably, there is amnesia (memory loss), which can be quite distressing to people such as not remembering that someone phoned while under the influence of the drug, you promised certain things, but you cannot remember the following morning what it was. Another problem is the development of addiction to the drugs with worse insomnia when the drugs are discontinued. Many physicians have stopped prescribing benzodiazepines.

2. There are non-benzodiazepines drugs that are used as sleeping pills (hypnotics), such as Zaleplon (Sonata), Zolpidem (Ambien) and Eszopiclone (Lunesta).  They seem to be better tolerated.

3. Ramelteon, a melatonin agonist, is available by prescription in the US. It probably is the best-tolerated mild sleeping pill and works similar to melatonin, but is more expensive. Chances are that your physician likely would prescribe one of the non-benzodiazepines drugs or Ramelteon for you as they do not seem to be addicting.

4. However, there is an alternative: Many patients with insomnia tolerate a low dose of trazodone (Desyrel), which is an antidepressant with sleep restoring properties. A low dose of 25 to 50 mg at bedtime is usually enough for insomnia. This allows the patient to fall asleep within about 30 minutes of taking it, and sleep lasts through most of the night without a hangover in the morning. Many specialists who run sleep laboratories recommend trazodone when primary insomnia is diagnosed. However, this is still a drug with potential side effects as mentioned in the trazodone link, but 50 mg is only ¼ of the full dose, so the side effects will also be less or negligible.

5. I prefer the use of melatonin, which is the natural brain hormone designed to put us to sleep. Between 1 mg and 6 mg are sufficient for most people. We know from other literature that up to 20 mg of melatonin has been used in humans as an immune stimulant in patients with metastatic melanoma with no untoward side effects other than nightmares and some tiredness in the morning. A review from the Vanderbilt University, Holland found melatonin to be very safe as a sleeping aid. There are several melatonin receptors in the body of vertebrates (including humans), which are stimulated by melatonin.

6. Other natural methods are the use of L-Tryptophan at a dose of 500 mg at bedtime, which can be combined with melatonin. It is the amino acid contained in turkey meat, which makes you tired after a Thanksgiving meal. GABA is another supplement, which is the relaxing hormone of your brain, but with this supplement tolerance develops after about 4 to 5 days, so it is only suitable for very short term use. Herbal sleep aids are hops, valerian extract and passionflower extract. They are available in health food stores.

Conclusion

A lack of sleep (insomnia) is almost a given in our fast paced lives.

When it comes to treatment, all of the other causes of secondary insomnia need to be treated or else treatment attempts would fail. What is left is primary insomnia. This is treated as follows:

We need to review our sleeping habits, lifestyles and substance abuse. Remove what is detrimental to your sleep. Start with the least invasive treatment modalities such as self-hypnosis tapes, melatonin, L-Tryptophan or herbal extracts. Should this not quite do the trick, asks your doctor for advice. The non-benzodiazepines drugs or Ramelteon would be the next level up. It may be that an alternative such as low dose trazodone would be of help. Only, if all this fails would I recommend to go to the more potent sleeping pills (keep in mind the potential for addiction to them).

Preserve Your Muscles And Joints

Our ancestors were hunters and gatherers, constantly on the go. They did not have to think too much about their muscle and joint health, they simply moved them. In our society this has changed a lot. At work we spend hours sitting at a desk, and then we use computers and watch television at home. Instead of walking to the neighborhood store, we use our car.

Here I will review what we can do to keep our joints and muscles in top shape until a ripe old age.

Brief intro regarding the anatomy of joints and muscles

Our joints are designed to give us full mobility. But the joints cannot do it alone. The muscles are designed to allow the joints to move in a full range. Without exercise the muscles will shrivel up (medical term “atrophy”) within only 2 to 3 weeks. So without regular exercise your joints won’t do you any good. Besides the joint capsules need regular stretching in full range exercises to produce the lubricating fluid (synovial fluid) that nourishes the joint surfaces and the menisci of the knees. Think of muscles and joints as being a functional unit designed to move you about.

Our joints have aerodynamic designs to do the most optimal job for our body. For instance the knees have more of a hinge design that includes menisci for shock absorption while the shoulders and hips have more of a ball and socket type construction.

Wear and tear with aging

It is usually thought that injuries and aging wear down the joints. But there are other factors such as the wide spread use of statins that can contribute to muscles weakness. Ironically statins are taken to protect the heart, but side effects can interfere with the ability to exercise your heart because of aching muscles and joints.

With optimal nutrition and avoidance of wheat and wheat products to prevent autoimmune arthritis (lupus, rheumatoid arthritis, dermatomyositis) your joints can stay young for much longer (explained further below). But your joints and muscles need to move through a full range of motion regularly to keep the blood circulation and nutrition of their tissues in top shape.

What causes joint deterioration?

Aging, weight gain, diabetes, smoking and lack of exercise all are known to cause a worsening of arthritis, particularly osteoarthritis, but also rheumatoid arthritis. The wrong diet with lots of sugar and starch and trans fats (hamburgers, pasta, sugar soda drinks) causes hyperinsulinemia (insulin overproduction, like in type 2 diabetes) and is almost guaranteed to make you sick with arthritis, obesity and diabetes.

There is also evidence that wheat causes inflammation and arthritis by stimulating your pancreas to produce too much insulin. This has been proven for dogs and for humans. A good diet book to follow is Dr. William Davis “Wheat Belly Cookbook” (Ref. 1) with 150 recipes. If you are overweight, these recipes will also help you to lose some weight effortlessly.

A caution to marathon runners: the constant pounding of prolonged jogging can cause osteoarthritis of hips and knees decades down the road. You may want to switch to different exercises before this happens.

What helps joints?

Molecularly distilled omega-3 fatty acid helps to prevent inflammation of your joints. Vitamin D3 will help your bones to be strong to support the tendons and ligaments. Chicken cartilage can build up joint cartilage within a few weeks! So, if you feel pain in your joints use 3 capsules of omega-3 (the strong, molecularly distilled ones) twice per day. This will help your joint inflammation within 3 to 4 weeks. If this alone is not enough add chicken cartilage from the health food store, which will help to build up the hyaline cartilage within your joints. For those who are questioning the effect of chicken cartilage, here is a 1993 chicken cartilage Harvard study proving it.

Below are more general steps that will help your joints, ligaments and muscles.

Maintaining health of joints and muscles

a)    It starts with good nutrition.

Hamburgers and deep fried French fries will not do the trick. Muscles require protein from meat, fish, poultry and dairy products. If you are a vegetarian you need to become knowledgeable on what essential amino acids are and what combination of vegetables will give you the amino acid composition to build up a full protein.

Joints need ingredients from cartilage, which you find in chicken cartilage (available in health food stores as fikzol (type II cartilage). I you prefer, chicken soup would also give you the ingredients to build up cartilage, but it would require a lot of regular chicken soup consumption to achieve this.

Sugar and starchy foods, which are broken down within half an hour after a meal into sugar in your blood, cause an insulin response from your pancreas. This in turn can cause inflammation in your joints and tendons. It is interesting to note that type 2 diabetes and arthritis are associated. A ketogenic, low sugar/starch diet will prevent arthritis and diabetes as it reduces the insulin level in the blood, which in turn turns off inflammation in the joints.

b)   Supplements:

Omega-3 fatty acids will help control any inflammation including the inflammation from arthritis (you need 3 capsules of the concentrated, molecularly distilled fish oil twice per day to achieve this).  DMSO gel, available in health food stores in the US, can also be used to rub onto inflamed joints. It will penetrate tissues rapidly, is nontoxic and helps control inflammation along with the omega-3 fatty acids. Regular anti-inflammatory pain relievers (NSAIDs) are harsh on your kidneys and can irritate the gastric lining causing bleeding gastric erosions, so definitely not recommendable.

Glucosamine, chondroitin sulfate, or a combination of both is available in the health food store and has been shown to help with osteoarthritis. I contributes to building up hyaline cartilage.

c)   Watch your weight:

It has been shown that the rate of degenerative arthritis (=osteoarthritis) in obese people is much higher when compared to slim people.

d)   Exercise:

You need to move your joints, ligaments and muscles every day to maintain their strength and range of motion. A daily workout at home or in a gym is best. I recommend 30 minutes of a treadmill or equivalent (jogging, Stairmaster etc.) as aerobic exercises. Then you need 30 minutes of isometric exercises like a circuit on exercise machines in the gym or dumbbells and expanders (resistance bands) at home. I consider this as the basic fitness routine every day.

Ballroom dancing and Latin dancing or Zumba is also a good combination exercise, which I would recommend on top of the basic exercise. Dancing helps to maintain your balance as well, which is something the older population tends to lose. In addition dancing stimulates your brain cells and makes you less vulnerable to develop dementia in old age.

Other aerobic exercises that can be recommended are walking (brisk walk) and/or intermittent jogging. Swimming has the advantage particularly for arthritis sufferers that you are floating. It allows you to exercise your leg and arm muscles, even if you have some arthritis pains.

e)  Pain relief: What could you do for pain relief? I do not like NSAIDs as this will damage your kidneys on the long-term and cause gastric erosions that can bleed massively. Electro acupuncture is very useful for muscle and joint pains and has no side effects. Physiotherapy treatments are useful to recondition your muscles and build up the range of motion of your joints. Chiropractic treatments for back and neck pain will also help. Instead of narcotics, why not try low dose Naltrexone (LDN). It has been shown to help with the pain of fibromyalgia.

Conclusion

In this brief review I have attempted to show you that your body is not on a one-way street in the direction of disability and death. There is a lot we can actively do to prevent this from happening prematurely. Just eat right, supplement (if you have symptoms), exercise and be active. Soon you will no longer be aware of your previously achy joints or muscles, as the pain tends to melt away when you are reconditioned.

Pimples And Acne Can Be Caused By Food

This article is about “pimples and acne can be caused by food”.For a long time nobody knew why teenagers get acne. But many assumed that it would come from hormonal changes as teenagers grow up. But why then are there some ethnic regions in the world where teenagers do not get acne? In this blog I will present the background that shows that wheat, sugar and dairy products are the culprits. People in regions of our planet where acne does not exist do not eat these foods.

Regions where acne does not exist

1. The Kitivan Islanders of Papua New Guinea have no cases of acne in teenagers. They adhere to the old hunter/gatherer diet of no sugar, no alcohol, no wheat and no grains. Instead they eat root vegetables such as sweet potato, yam, taro, tapioca; fruit like papaya, pineapple, banana, mango, watermelon, guava and pumpkin; and also vegetables, coconuts and fish.

2. African Bantus and Zulus: These original African warriors eat a low glycemic diet with no wheat, no milk and no refined sugar or starches. Their teenagers and young adult do not have acne, if they stick to the original tribal diet.

Further regions without acne

3. Aché hunter/gatherers of Paraguay: a study by researchers from the Colorado State University in 2002 showed that sugar, wheat and other high-glycemic foods were missing in the diet of these native tribes. As a result they have no acne when they consume this type of diet, which is very similar to the Kitivan Islanders of Papua New Guinea.

4. Japan’s Okinawans when sticking to their original diet before 1970 had clear complexion and no pimples (acne). But as this link shows the McDonald’s and other fast foods with too much salt, too much sugar, wheat, deep fried and convenience foods entered the scene after 1970 and the acne rate went up to the American level.

More regions without acne

5. The natives of the Purus Valley in Brazil: A dermatological examination of 9955 school children age 6 to 16 showed an acne incidence of only 2.7%. In contrast in Westernized countries the rate of acne is 60 to 80%. The diet in this region is again similar to the other groups already mentioned above.

6. Canadian Inuit before 1950 did not consume dairy products and were acne free. Since then, there has been a steady increase of dairy products, soda, beef, and processed foods.

How acne develops

The medical term for pimples or acne is “acne vulgaris”. For years it has been postulated that hormones and medication can cause acne. According to Ref.1 there are several steps that work together in causing acne. The hair follicle and sebaceous gland work as one unit. Male hormones, called androgens play an important role in the development of acne, both in males and females. Males produce testosterone not only in testicles, but also in the skin itself. An enzyme, 5-alpha-reductase, converts it into the much more active metabolite dihydrotestosterone. In individuals with hypersensitive receptors in the sebaceous gland this causes blockage in the sebaceous gland ducts. At the same time, it stimulates the sebaceous gland oil production leading to the formation of a keratotic plug. White heads and black heads are formed this way.

Factors leading to inflammatory substances

Factors that contribute to inflammatory substances are sugar, wheat and starch intake causing insulin release. This stimulates IGF-1 receptors in the skin, which causes growth of the subcutaneous skin layers, which is pushing up from the layer below the skin, kinking the sebaceous gland duct and causing acne pustules (pimples) to form. A skin bacterium, called Propionibacterium acnes (P. acnes), is getting trapped in the pimple causing a local skin infection, which in turn can cause acne cysts and furuncles, particularly in males where there is a family history of acne. High cortisol levels from stress can also be a contributing factor in causing acne. Today’s teenagers are exposed to a lot of stresses from exams, competitive sports and peer pressures.

Polycystic ovary syndrome

Females with PCOS (polycystic ovary syndrome) have higher androgen production from ovarian cysts, which results in acne as well.

Both male and female teenagers experience an androgen surge when puberty sets in. If the teenager avoids the additional insulin response, which comes from eating sugar, starch, grain and particularly from consuming wheat and wheat products, the plugging up of skin pores will not occur, meaning these teenagers will be acne free.

Milk sensitivity

Some teenagers are also sensitive to milk protein from milk and milk products. In sensitive people whey protein allergy causes the same insulin/skin IGF-1 response described above. This leads to blocking of skin pores. If there is no blockage in the hair follicle, the P. acnes bacteria will stay on the surface of the skin (these bacteria are part of the normal skin flora) and the sebaceous gland secretions flow unimpededly to the surface of the skin keeping it naturally lubricated. These observations are further confirmed by a study from Malaysia in 2012 showing that a high glycemic load diet with milk and ice cream caused worsening of acne in teenagers of both sexes.

Treating acne correctly

A)   Conventional acne treatment

Big Pharma has a firm hand in the treatment of acne;  they are supporting symptomatic treatment of acne rather than treating the cause. There are surface treatment modalities that open the skin pores: peeling agents such as benzoyl peroxide. General practitioners often treat the infection with antibiotic pills (tetracycline or erythromycin), but this is not treating the cause, only the super infection that comes from the plugged-up skin pores (stasis of sebaceous gland secretions). Another approach is topical application of antibiotic and peeling agent in combination (1% clindamycin and 5% benzoyl peroxide gel), which the patient applies twice daily (Ref.2). Resistant cases, usually the ones who have a family history of severe acne, have been treated by a skin specialist who has a special license to treat with isotretinoin (Accutane), a vitamin A derivative. It works in many cases, but it often has serious side effects.

Side effects of Accutane

These include skin dryness, eye dryness, muscle and bone pains, headaches and liver enzyme abnormalities. There can also be an instability of mood including depression and causing birth defects in the fetus of a pregnant woman (Ref. 3). In 2009 the manufacturer stopped distributing the drug in the US, because of too many lawsuits regarding damages from the drug.

I am not saying you should ever take this toxic medication. What I am saying is that treating symptoms, but not the cause has led to peculiar drug manufacturing. Physicians use this drug now to treat brain cancer and pancreatic cancer.

B)   Dietary approach to treat acne

There has been a renewed interest in the last 40 years to sort out the connection between dietary factors and acne. The most straightforward treatment in my opinion is to modify what you eat.

A clinical trial from the University of Melbourne in 2007 showed that a low-glycemic diet reduced the acne lesions by 22% compared to a control group.

Two factors are clear: a low-glycemic diet produces fewer pimples, the stricter the patient applies a low-glycemic diet, the more effective the treatment is. Up to 50% reduction in acne lesions were observed among patients with acne who adhered to a strict low-glycemic index diet in just 12 weeks. There is also evidence that milk and other dairy products can contribute to acne, which works through the same mechanism of IGF-1 stimulation mentioned above.

Milk and skim milk can cause acne

A US study from Boston showed a 22% increase in acne lesions with total milk consumption and increase of 44% after skim milk consumption.

Omega-3-fatty acid supplementation is useful for inflammatory acne in about 2/3 of the cases as this study showed. Here is an image of a patient from this study who benefitted from omega-3 supplementation. The baseline image is seen with inflammatory acne lesions on his cheek. Only 12 weeks after taking 3 Grams of omega-3 supplementation daily his face looked much improved.

Conclusion

There is a lesson we learn from the analysis of the regions in the world where acne does not exist. Cutting out wheat, wheat products, grains, sugar, milk and milk products leads to amazing results regarding acne prevention. This leads to improvements of patients who suffer from acne. We have been lulled into believing that medical science will give us a magic pill to solve our complexion problems. I mentioned that one of the “magic pills” (isotretinoin) is so toxic that physicians now use it for cancer treatments. All along we allowed the food industry to destroy our complexion. They did so by inducing an insulin and IGF-1 response that plugged up our skin pores. We can open them up by eliminating certain foods. They are wheat and wheat products, sugar, high-glycemic foods as well as dairy products.

The Inconvenient Truth About Convenience Foods

When your grandmother grew up there was very little convenience food, maybe ketchup and yes, there was processed cheese and coke. There were also bread and butter.

Now we go through a large grocery store and the center of the whole store is occupied by convenience food, row after row.

What is convenience food? It is pre-cooked or processed food that sits on a shelf waiting to be bought and consumed. You may be able to just eat it the way it is (power bars, fruit yoghurt snacks, ice cream, breakfast cereals etc.) or you just have to microwave it for a minute or two (ready made meals, pizzas). Even, if you make a fresh salad, you top it with a salad dressing that has been processed and may contain chemicals that are not necessarily healthy for you.

This blog is meant to make you think and get educated as a consumer. As a physician I am guided by what is healthy for you, but at the same time food needs to be interesting and taste good and be affordable.

As fat, carbohydrates and protein are the main food groups that we eat, I will deal with each of these categories first followed by vitamins and minerals, which we also need.

Fats and oils

Many convenience foods are full of saturated fatty acids, which contribute to the overall calorie count of the package and are one of the main reasons why we gain weight and deposit fat into our arteries in preparation for a heart attack or stroke down the road. As you may know the worst form of fat is hydrogenated fat, also known as “trans fat”.

It contains free radicals from the hydrogenation process, which damage your cells and interfere with normal body metabolism. Read labels and avoid any foods that have a long shelf life as this is due to hydrogenated fats and chemicals known as food preservatives.

This food group also contains sausages and other processed meat; I wrote a separate blog about this recently.

If you eat cheese, reduce your saturated fat intake by buying cheese with only 18% fat (such as Cantenaar cheese, Jarlsberg light, skim milk mozzarella and goat cheese). Avoid the rich 45% type cheeses. The best oil in your kitchen would be an organic cold pressed olive oil. It figures prominently in Mediterranean cooking.

Sugar, starch and other carbohydrates

A large portion of snacks from the mid section of the grocery store contains all forms of sugar: high fructose corn syrup, sugar, honey, agave syrup, maple syrup etc.  You may think that a harmless fruit juice would be healthy until you see from the ingredient list on the label that it contains 5 to 6 teaspoons of sugar per cup (250 ml) of juice.

Unfortunately our body is not equipped to process all the sugar that the food industry wants us to consume and we develop insulin resistance; the liver converts the excess sugars into fat and deposits it into our arteries and as fat deposits between our guts (visceral fat) and as subcutaneous fat in the thighs, around the hips and the waist. It is no secret that a lot of obesity is related to overconsumption of sugar containing convenience foods (snacks and sugar-laden drinks).

Often low calorie alternatives contain aspartame or sucralose (Splenda). Aspartame is an excitotoxin damaging your brain cells and sucralose was developed in the 1950’s as an insecticide. We do not want to replace disease-promoting sugar with toxins as sweeteners. Safe alternatives for sugar are xylitol, mannitol, and stevia.

What is sometimes overlooked is the fact that your body digests bread, starchy foods such as potatoes, and pasta, rice and flour products like pizza or cookies within 30 minutes into sugar that is as harmful to your pancreas as plain sugar or high fructose corn syrup. The body reacts with the same overproduction of insulin converting the excess sugar into fat and depositing it in your body as described above. Much of the obesity wave we see in the past 3 decades is due to baked goods like bagels, bread, pasta and pizza. It is much better to enjoy your stevia-sweetened coffee without any bakery pieces to go along with it.

Protein in meats, dairy products and sausages

You would think that a healthy cut of meat from the grocery store would be a good source of protein for you. You probably did not think that it could be contaminated with a superbug when you bought it. This is especially true for ground meats like hamburger meat. If you bought a portion of organic meat you can be more certain that you are buying a qualitatively superior product. I discussed this whole issue of superbugs in meat and meat products in this blog recently.

We need to be aware of the agroindustry, the feedlots and what they fed the animals. I only buy organic meat and organic dairy products as my source of protein. I avoid sausages altogether because of the food additives that they contain, which are cancer causing.

The problem with prepared meats like chicken nuggets and others is that they contain breading and food preservatives and they have been deep fried, which makes these items an unhealthy choice.

What are some of the problems with dairy products? Despite the allegations that bovine growth hormone would be harmless to your health, your body thinks otherwise. Your body has hormone receptors that are very specific and bovine growth hormone can block them so your own human growth hormone from the pituitary gland cannot function properly. This is why I would recommend only organic milk products. You may have heard that in many European countries bovine growth hormone is banned for that reason.

Next the fat content of dairy products needs to be monitored: go for low-fat milk, cheese and yoghurt. While we are talking about yoghurt, stay away from fruit yoghurts that have all kinds of sugar and food additives mixed in. Add fruit of your choice and stevia, if you need a sweet taste.

Vitamins and minerals

The more foods are processed, the less natural vitamins and minerals stay behind. Particularly vitamin C and the B complex group are affected, but also magnesium, which is an important co-factor to enzymatic reactions within our cells. Often processed foods contain too much salt with sodium displacing potassium from the cells resulting in a lack of energy and high blood pressure.

Your best prevention is to stick to as little processed food as possible and to eat organic. If you eat enough organic greens and vegetables, there is an ample supply of vitamins and minerals. Prepare your own soups as canned products are high in sodium; another unwanted additive is often sodium glutamate (MSG), which comes under many disguised names. It belongs to the group of excitotoxins like aspartame and is not welcomed by your brain cells.

Public Awareness

Lately there has been more of public interest and awareness to the detrimental effects of convenience foods. Alarming reports about the increase in the obesity rates, the rise in diabetes type 2 even in children have been in the media for some time. The publications are not only North American, but also European, as can bee seen in this link.

New legislation is being introduced in many states of the US regarding school snacks and vending machines in schools.

Not all food news is bad. Recently it was reported that fish oil could protect against the effects of junk food. Omega-3-fatty acids contained in fish oil are helping to rebalance the ratio between omega 3 and omega 6-fatty acids in food, which often is disbalanced towards an overabundance of omega-6 fatty aids in processed foods. Rebalancing the omega3/omega6 ratio in food helps to normalize the metabolism of the brain and prevents hardening of arteries.

What you can do to get healthy food

It starts when you buy food. Read labels and look for calories, sugar, fat and sodium content. You may be surprised how many stores carry organic foods now. The price may not be that much more. There is a useful app for your cell phone, Buycott, that you may want to download. This way you can scan items in the store and find out what ingredients are contained in a particular food item and which company produces it.

With meats it is particularly important to buy organic (because of superbugs and also because of the aspect that feed lot animals often receive antibiotics and hormones). Stick to organics also with vegetables and greens (xenoestrogens in non-organic greens that block hormone receptors). Milk products also need to be organic because of the bovine growth hormone facts mentioned above.

When you eat out, things become more difficult unless you find an organic food restaurant. You can always prepare your own salad for lunch with organic greens and a lean protein food, which you keep refrigerated until you are ready to consume it. On weekends a portable picnic in a park can be a great way to relax and socialize, especially in summer.

What We Can Do About Superbugs

Introduction

This article is about what we can do about superbugs. Several decades ago nobody talked about superbugs. Then came stories of “flesh-eating disease” or necrotizing fasciitis. Since 2001 it became obvious that these cases have become more common. Methicillin-resistant Staphylococcus aureus (MRSA) was often the underlying cause.

Antibiotics for weight gain in cattle

Government bodies accuse physicians of overprescribing unnecessary antibiotics for viral colds and flus. All physicians had to attend educational programs as part of the continuing education programs to use antibiotics only sparingly.  But the frequency of these serious infections often requiring amputations and mutilating surgeries continued to escalate. Research into the phenomenon of increasing superbugs took place already in the 1970’s. In 1977 the FDA came to the conclusion that antibiotics were used widely in the US by the agroindustry for weight gain in livestock (pigs, cattle, chicken, turkeys). Although masked as protecting animals from infections, the real motivation of the farmer was to increase profits. The FDA recommendation in 1977 to change the practice of feeding livestock antibiotics did nothing to change that.

After a court review in 2012 a New York court ordered the FDA to do something about the same problem. The answer was a lame recommendation of a voluntary program to downsize the use of antibiotics in livestock, requiring a vet and a prescription for antibiotics. The problem with this is that farms had no mandatory checks of livestock by trained inspectors. The ruling about antibiotic reduction contained no penalties for farmers who continued the old antibiotic feeding practice. Fast-forward to an American citizen who visited India recently as told in this story.

Superbugs are all over the world

It is clear that superbugs are all over the world. It is also clear from this article (and other literature I have reviewed) that 80% of today’s antibiotics are fed to livestock, not to treat infections, but because of their effect on weight gain in livestock and the associated larger profits. The result is that we are looking at farming practices that produce deadly superbugs. Next we are reading headlines about a recall of meat meat and meat products.

Here is an interesting list from the Environmental Working Group, which shows what percentage of meats in your neighborhood grocery store is contaminated, and these are the numbers for superbug contaminations: The worst is ground turkey with 81%, pork chops with 69%, ground beef with 55% and chicken breasts, wings or thighs with 39%.

Wash meat thoroughly

This shows how important it is to wash meat thoroughly and to cook it long enough to kill the superbugs. It is also extremely important to frequently wash your hands when you prepare meat. This avoids  colonizing your skin surface with superbugs. The first step is for a person to have skin surface flora with superbugs. The second step is to get a small abrasion or a skin sore where a superbug can enter. The final step is that this bug multiplies under the surface of the skin and starts a serious infection. If the immune system is not in top shape to eradicate these bugs right away, the next step may be flesh-eating disease or toxic shock syndrome.

Solutions to stay on top of Superbugs

In northern Saskatchewan, one the provinces of Canada, an 8 year community based study was done to see whether it would be possible to reduce community-associated methicillin-resistant Staphylococcus aureus (CA-MRSA) by a combination of hand washing and education regarding appropriate antibiotic use. Using this program it was shown that in the time period of 2006 to 2008 the infection rates went down from 242.8 to 129.3 infections/10,000 population, which is almost half of the infection rate from before.

Hospital associated methicillin-resistant Staphylococcus aureus

What can physicians and nurses do about hospital-associated methicillin-resistant Staphylococcus aureus (HA-MRSA) and other superbugs in the hospital? In a hospital it is extremely important that HA-MRSA free patients do not get exposure to this superbug . Their immune system is already weak from the underlying disease. This is how they got into the hospital. At Stanford University a robot was developed that emits pulsating ultra violet light to kill any leftover superbug after the initial cleanup with scrubbing using bleach and germicidal solutions. Support staff treat the isolation rooms with the double treatment and robots disinfect the operating rooms overnight. So far tests have shown a complete eradication of the superbugs with these methods.

Breaking the chain of infection

Since about 2005 the major food production animals have been shown to be colonized with superbugs (MRSA) as this publication shows.

With 80% of the world’s production of antibiotics still going into the agroindustry for weight gaining purposes, it is high time that international “laws with teeth” apply. The WHO needs to get action in this and the public needs to put pressure on politicians to achieve this. Medical history has shown that infectious epidemics can be cured by breaking the chain of infection. In the example regarding superbugs the story of interrupting the chain of infection is exactly the same here: As shown in this study from Stanford University MRSA bugs are not contained in organic foods. So, by attempting to eat 100% organic, which can be challenging at times, you can intercept the infectious cycle involving MRSA and other Superbugs.

Attempt to eat mostly organic foods

You do not get exposed to the meat from food production animals treated with antibiotics for growth purposes. In the meantime, you are protecting yourself by keeping fit, taking vitamins and supplements all with the hope that the immune system stays strong from this. By eating organic meats and meat products you also keep your body free of toxins that would weaken your immune system and set you up for getting autoimmune diseases down the road.

The end result is that people who follow this example avoid colonization from exposure to superbug-infected meat products.  At the same time this prevents exposure to other toxins that a manufacturer of organic food can not use. Here is a story of a company that produces meat products without antibiotics (organic meats).

Conclusion

You can interrupt the infectious cycle of superbugs by eating organic foods. Make sure your immune system is strong by exercising, supplementing with vitamins and minerals. Wash your hands with soap and clean water, particularly after handling meat or meat products. Do not take antibiotics for a long time unless there is a specific reason of  bacterial infection that requires it. Do not consume meats from animals that were fed antibiotics. Read food labels!

Treatment For Alzheimer’s Failed, But Prevention Succeeds

Recently another news story about a failed drug against Alzheimer’s disease (AD) went through the news media as shown in this link.

Donepezil, galantamine, rivastigmine and memantine are the most common drugs used to attempt to treat Alzheimer’s as this review explains. None of these drugs are a real breakthrough with regard to truly curing AD, as the drugs only achieve a few months of delay in the eventual deterioration of the AD patient’s symptoms. On the other hand there is an overwhelming accumulation of data in the last few years showing that many different factors can prevent AD and dementia. Below I am reviewing all these preventative factors and steps.

Genetic and epigenetic factors in Alzheimer’s disease

Early onset Alzheimer’s disease occurs between 30 and 60 years of age. It is due to a genetic predisposition (mutations on genes of chromosomes 1, 14 and 21). Only about 5% of all AD cases are caused this way. The remaining 95% of Alzheimer’s cases are due to late-onset Alzheimer’s disease. Here the causation is due to a combination of genetic, environmental and lifestyle factors. One genetic risk factor in this group is important, namely the apolipoprotein E gene (APOE), which is located on chromosome 19. There are several forms of APOE as this review explains. It also states that there is so much variation between the various APOE forms and even the worst form of this does not necessarily mean that the person who has this will come down with late-onset Alzheimer’s disease. So APOE is presently only used in research projects. Your doctor will only order genetic tests in people who have a strong family history of early onset AD.

There is another genetic marker, the CYP46 gene that was found to be present in some late-onset AD patients. If it is combined in a patient with the APOE gene, there is a much higher chance of developing AD as this review shows.

Epigenetic factors are probably more important than genetic factors for most cases of late-onset AD, as this review explains. Another review came to the same conclusion.

What are epigenetic factors? Exercising, replacing missing hormones, using a calorie restricted, only 15-20% fat containing diet; and taking supplements as listed below that will keep harmful genes in the “off” position and protective genes in the “on” position. Taking these preventative steps is probably more powerful than using any of the presently available medications mentioned above.

Exercise, diet, control blood pressure

As already mentioned, these are some of the powerful epigenetic factors that will prevent AD down the road. Controlling blood pressure has long been known to improve cognitive function. It is now evident that there seems to be a problem with microcirculation in brain tissue before it comes to neurodegenerative changes of AD and the underlying deficiency in nitric oxide production in the lining of the diseased arteries. Other research has shown that a lack of nitric oxide (NO) production is also the underlying problem with hypertension.

Green vegetables such as kale, spinach, also cabbage varieties and red beets are a source of nitric oxide and have also been shown to prevent AD at the same time.

Add to this exercise and you have a winning combination for the prevention of AD. You guessed right: exercise increases NO production from he lining of your arteries. When people age their lining of the arteries does not produce as much NO as in younger years. However, there is a supplement available, Neo40 Daily, that can be taken twice a day to compensate for this.

Here is another report about a 30% to 40% reduction in the incidence of AD when people do regular, simple exercises.

More good news about fruit and vegetables: tomatoes, watermelons, pink guava, pink grapefruit, papaya, apricot and other fruit all contain lycopenes, which have been shown to prevent AD.

Recently a new testing tool in combination with a PET scan of the brain has been developed, which may help the treating physicians to assess improvement or deterioration of an AD patient objectively using this method. However, this is still considered to be only a research tool at this time.

Supplements to prevent Alzheimer’s disease

The following brain-specific nutrients play a part in the prevention and treatment of AD (according to Ref.1):

1. B-vitamins: they are important to support the energy metabolism of brain cells.

2. Vitamin C: this has antioxidant properties and prevents brain cells and supportive glia cells from oxidizing.

3. Vitamin E in the form of mixed tocopherols: together with vitamin C has been shown to prevent Alzheimer’s disease

4. Phosphatidylserine (PS), with an intake of up to 300mg/day: counteracts and prevents memory loss.

5. Coenzyme Q10 (ubiquinone), 100mg/day (it would be safe to take 400 mg per day, which is also cardio protective): stabilizes the mitochondria of brain cells and heart muscle cells. It is a powerful neuroprotective agent and supports ATP production (energy metabolism of brain cells).

6. Ginkgo (Ginkgo biloba), at a dose up to 240mg/day: increases micro vascular circulation, neutralizes free radicals from oxidation and improves short-term memory.

7. Omega-3 fatty acid and DHA, 1500mg/day: has anti-inflammatory properties.

Other nutrients that hold promise are:

8. Huperzine A, 100 to 200mg/day: natural anticholinesterase inhibitor, derived from the Chinese club moss, surpasses donezepil according to studies by doctors in China

9. Vinpocetine, 2.5 to 10mg/day: comes from the periwinkle plant, increases cerebral blood flow and stimulates brain cell metabolism

10. Turmeric extract (curcumin) is very beneficial in reducing tau protein deposits in AD.

All these statements and dosages are cited from Ref.1.

Hormones to prevent Alzheimer’s disease

According to Ref. 1 there are certain hormones that can prevent AD: DHEA, pregnenolone, estrogen (bioidentical estrogen only).

  1. DHEA is persistently low in AD patients and replacement with DHEA at 50 mg daily has shown improvements in muscle strength and energy of AD patients.
  2. Pregnenolone has been shown to be a powerful memory enhancer in animals and humans alike.
  3. Estrogen, if taken as bioidentical estrogen cream (Bi-Est) can improve brain function. Estrogen is a strong epigenetic switch that keeps a woman mentally younger for longer, but has to be balanced with bioidentical progesterone cream to prevent breast cancer and uterine cancer. A study showed that estrogen replacement early in menopause will cut down on the heart attack rates, but it is also known, particularly when given as bioidentical hormone cream to prevent AD.
  4. In addition progesterone has been described to be of value in the aging woman to preserve brain metabolism.
  5. Testosterone is known to protect against Alzheimer’s disease in the aging male.
  6. Melatonin at a starting dose of 1 mg to 3 mg at bedtime often helps to restore the disturbed sleep pattern, but also augments the effects of the other hormones (Ref.1).

Removal of toxins, particularly mercury

Mercury is extremely toxic in minute amounts and affects brain cells preferentially. Intravenous vitamin C/glutathione treatments as described in this blog will remove mercury from your system including the brain.

It may take 20 to 30 such treatments in weekly intervals followed by a maintenance program every two to three weeks to remove mercury from the body.

Other heavy metals can accumulate in the brain as well and must be removed. This is described here in more detail.

Conclusion

There have been major breakthroughs in prevention of Alzheimer’s disease and dementia over the past few years, many unnoticed by the media. The search is still on for an effective drug that would treat AD when it is present. However, this may be 10 or 15 years away and we cannot afford to wait that long. Instead I suggest that people should embrace the concept of preventing AD by using as many of the factors described above. Both at the 2011 and the 2012 Anti-Aging Conferences in Las Vegas several speakers pointed out that a combination of several preventative factors will be much more effective than one factor alone and they estimated that about 80% of AD could be prevented this way.

The recent news of gut bacteria possibly being involved in causing heart attacks (see details below) sounds pretty outrageous. In order to make this more understandable, we need to start backtracking how bacteria end up in our gut and what they are doing there.

Gut bacteria in our bowels at various stages in life

Even among infants there is a huge difference in the composition of the gut flora depending on whether the baby was vaginally delivered, born by Cesarean section or prematurely born and nursed in an intensive care unit for a prolonged period of time.

When we are grown up, it depends on where we live on our planet as to what type of gut flora we have. A 2011 study of the bacterial composition of stool samples in Europe versus Africa showed that the European microbiome in the colon was typical of the Bacteroides enterotype, whereas the African microbiome was dominated by the Prevotella enterotype. Other studies have shown that the type of diet we eat causes us to harbor different types of gut flora. A person eating a more vegetarian diet will have a Prevotella enterotype gut flora, while another person eating a Western style fat and protein dominated diet will have a Bacteroides enterotype gut flora. What is the gut flora doing? It lives within our gut for our mutual benefit. We provide the gut bacteria a comfortable home at body temperature where they grow best. They in turn ferment undigested foods, protect us from the growth of harmful bacteria, produce vitamins like vitamin K and biotin, and produce hormones that direct the body to store excess food as fat.

Cleveland Clinic’s gut flora research linked to heart attacks

In April 2013 reports that heart attacks may be related to the composition of your gut flora hit the popular press, articles like this. Two years earlier, in April of 2011, the same Cleveland research group under Dr. Stanley Hazen found that the gut flora composition has an impact on heart disease. They stated that a “byproduct called trimethylamine N-oxide (TMAO) is produced when intestinal bacteria digest the nutrient phosphatidylcholine, commonly known as lecithin”. It was determined that the liver was responsible for producing TMAO out of absorbed materials from gut bacteria and that this was responsible for accelerating hardening of the arteries, which in turn caused deadly heart attacks and strokes.

As can be seen from this link the Cleveland research group is suggesting that TMAO could become a valuable screening tool when measured in patients’ blood to screen for their risk to develop a heart attack or stroke in the future. Furthermore, once these patients are identified, they can be sent to a dietitian and be put on a different diet that prevents heart attacks and strokes; also, the progress could again be monitored through repeat TMAO blood tests down the road.

Criticism of research from the Cleveland Clinic

Not every one has welcomed the study as clear-cut proof for a link between the gut flora and heart attacks, strokes and deaths. This link shows that it is questioned whether perhaps low antibiotic residues in red meat would explain the findings of the study. It rightly points out that there was no control for whether the more than 4000 patients were eating organic foods or a regular American diet. The comment goes on to say that the regular American diet contains residual antibiotics from milk, milk products, beef, chicken and turkey. These traces of antibiotics are powerful enough to have an effect on the gut flora, which likely is the reason for the differences between the vegetarian diet versus a Western style fat and protein dominated diet. It is entirely possible that grass fed beef would not lead to TMAO levels in the blood whereas regular beef or hamburger meat will raise the TMAO levels. The same could be true for egg consumption, which was also accused of raising TMAO levels. However, we do not know whether organic eggs that do not contain antibiotics would have done the same as regular eggs that contain traces of antibiotics.

Cancer caused by gut flora

This 2008 review article explains how lactobacillus and bifidobacteria (probiotics) prevent colon cancer, while bacteria of the Bacteroides and Clostridium variety increase the incidence of colon cancer and the growth rate of colonic tumors. So, the composition of your gut flora does not only matter with regard to prevention of heart attacks and strokes, but is also important in preventing colon cancer.

Change your gut flora to boost your health

Here is where the “rubber hits the road”. What I mean is that you can benefit from all of this research with regard to your health by paying attention to a few facts and possibly making a few changes as follows.

  1. I think that the argument of the bulletproofexec.com website holds true and we should all eat organic meats whenever possible. The Cleveland study has already shown that the gut flora in vegetarians stayed healthy even when they occasionally slipped and ate a regular steak.

  3. Higher intake of fruit and vegetables make a measurable difference in the body decreasing the risk for heart attacks and strokes as this British research group has shown. Another research group from Italy has confirmed that an increased intake of fruit and vegetables mobilizes genetic switches that will stabilize the metabolism of the gut wall and prevent cancer of the colon.

So, the verdict for boosting your health is out: eat organic foods, use probiotics as supplements and eat more fruit and vegetables!

This article is about “get rid of toxins safely”. You often hear that people use chelation at home with natural foods. Is that true?

The standard chelation detoxification

I like to mention in the beginning that the proven way to reduce heavy metals in the blood (typically mercury and lead) is by intravenous chelating agents such as EDTA. However, the lowering of calcium levels as a severe side effect limits the application of it. There are also blood-thinning side effects to consider. EDTA is a chemically synthesized compound, not a natural substance. Conversely, vitamin C and Glutathione are both antioxidant substances that the body knows; they are non-toxic and can be used more easily intravenously to remove heavy metals like lead and mercury with fewer side effects than EDTA.

Natural ways to use chelation in order to detoxify

Although I think that EDTA has a place in the hands of those who are knowledgeable about the side effects, the vitamin C/Glutathione combination is probably a more physiological detoxification method as glutathione is synthesized by the liver to detoxify our bodies. Intravenous chelation is expensive, requires access to a vein and may not be a suitable way for many people to detoxify. There are natural ways to detox for those who cannot afford chelation or cannot chelate for other health reasons.  The major detoxification organs are: the liver, the kidneys, the gut, the lungs and the skin.

What about oral methods to detoxify?

Several methods of cleansing are fasting, the major cleanse or the raw food cleanse. I would not advise to do strict fasting without fluid intake as it takes fluid intake to be able to dissolve any released toxins into urine, feces, invisible fluid from exhaled air and nasal discharge. Next there is the “master cleanse” with lemon juice, maple syrup and cayenne pepper as indicated in the link above. I feel differently about a raw food cleanse as I will explain. First, I like to point out that raw food is not easily digested as your gut absorbs only about 30 to 40% of the nutrients. (There are several brands like the BlendTec or the Vitamix.) The powerful blending process breaks up the cell walls and releases the content of the plant cells including their enzymes and nutrients, so that 80% of the nutrients are absorbed in your gut.

Are there supplements that can help you to detoxify?

As Dr. Mercola describes that in Japan chlorella powder is one of the most popular supplements to detoxify. It is difficult to get a complete, non-biased picture about chlorella. I did an extensive research in MD Consult about the scientific evidence for detoxification by chlorella in humans. Unfortunately, the search results came back only for experiments with mice and rats, but there were no human detox results.

Perhaps the Wikipedia description about the history of algae and chlorella as a possible food supplement sheds some light on the difficulties getting real hard data on chlorella detoxification. Researchers poisoned rats deliberately with cadmium (a heavy toxic metal). There was no evidence of detoxification with chlorella when the researchers stopped cadmium poisoning. However, the article goes on to say that chlorella did prevent absorption of cadmium from the intestines to a certain extent (in rats).

Liver detoxification and colonic detoxification

Dr. Max Gerson invented coffee enemas many years ago when he treated end stage cancer patients with natural methods. He found that when he detoxified them with raw juices, many cancer patients died from the toxic breakdown products of dying tumor cells. So he was in dire need of a procedure that would remove the toxic tumor substances efficiently.  This is when Dr. Gerson used coffee enemas. As the link above describes, coffee given through the colon as an enema (we would perhaps call it “coffee colonics” today) stimulates the liver and the bile flow thus detoxifying the blood effectively of toxic materials. In addition, the secretory capability of the large surface of the colon wall also helps the detoxification process. Juicing in combination with coffee enemas improved Dr. Gerson’s end stage cancer cure rates dramatically.

Dr. Kelley introduced supplementation with Mega pancreatic enzymes

Subsequently, Dr. Kelley developed his nutritional-metabolic therapy for end stage cancer patients where he added large amounts of pancreatic enzymes in addition to coffee enemas. Dr. Denis Burkitt is credited for recognizing the importance of fiber in the diet. He noticed that there was a striking difference between the colon cancer rates in England versus the rates in Africa. He analyzed food habits in both locations and found that in England there was not enough fiber consumption.

Diet high in vegetables, fruits and whole grains reduces colon cancer rates

A large study done by the Harvard Medical School showed that it was not so much the fiber that helped to prevent colon cancer. Further large scale studies were done and it was shown that a diet high in vegetables, fruits, and whole grains was what was responsible for a reduction in colon cancer rates. This type of diet will also help detoxify your system from carcinogenic substances in general and from other toxins as the soluble fiber inside the gut will attach to it and go down the toilet instead of being reabsorbed in the colon into your system causing cancer.

Skin detoxification

The skin is the largest body organ with a lot of skin surface where skin pores can help detoxify under the right circumstances. You can achieve this with infrared saunas, with exercise that leads to production of sweat and by soaking in Epsom salt baths. Traditionally this has been done for centuries in the European spas, originally described by Sebastian Kneipp. The medical establishment in Europe (France, England, Germany, Austria), unlike American physicians, embraced the concept of natural therapies under the term “balneology”, at least for a number of decades. But  overuse by patients (abusing the system), badmouthing the relatively weak effect of detoxifying by Big Pharma and lack of funds from the health insurance companies put a damper on his movement. Part of this approach was the use of skin detoxification with healing baths (using Epsom salt and other salts) and physical exercise to the point of sweating.

In addition, “Wechselbäder” according to Sebastian Kneipp (alternating hot and cold baths) are also useful to stimulate the circulation and the immune system. The major component of Epsom salt is magnesium sulfate, which can be absorbed through the skin to a certain degree. So, some of the healing effect of Epsom salt baths may have come from an indirect balancing of low blood magnesium and the normalization of many metabolic processes that require magnesium as cofactors. Here is another, more vernacular description of Epsom salt use. Some of the assumptions such as improvement of diabetes, improvement of digestive absorption or relief from migraine headaches may be a combination of exaggeration and placebo effect rather than reality.

Kidney detoxification

The kidneys along with the liver are the major, natural detoxification organs of the body. However, they need enough fluids to dissolve water-soluble toxins and eliminate them from the body through the urine. Consume clean water intermittently during the day (reverse osmosis water in glass bottles or BPA free bottled water). Because alcohol is a cell poison damaging the vital organs like the brain, heart, liver and kidneys it is best to minimize any alcohol intake to the absolute minimum (best to avoid alcohol altogether). The main filtration devices in the kidneys are the glomeruli, little packages of convoluted capillaries, which filter the blood several times per day.

Too much salt and sugar interfere with normal glomerulus function

High salt intake, too much sugar and too much starch interfere with normal function of the glomeruli. Insulin resistance in cases of metabolic syndrome and diabetes (often in people with obesity) can damage the kidneys.  In this case the filtration devices of the kidneys suffer damage and cause toxin accumulation. To prevent damage of the kidney capillaries you need a healthy, basic diet like the Zone diet, South Beach diet or the Mediterranean diet as mentioned in this link.

This will keep fat deposits out of your arteries and maintain the capillaries of the glomeruli of your kidneys. This means that the filtration process continues to work until a ripe old age. Your doctor can order tests (creatinine clearance) to assess whether the filtration capacity of your kidneys is normal.

Many toxins are fat-soluble

Many toxins are fat-soluble and store in the fatty tissues. When you lose weight, you need to drink more fluids to get rid of the water-soluble toxins. This occurs through the kidneys. You can also use coffee enemas to rid yourself of fat-soluble toxins (through bile and bowel movements).

There are a lot of websites that want to sell you herbs that should cleanse your system. They have a marginal effect. But I like to stress that you want to adopt one of the diets mentioned above first. This starts you with a healthy baseline. Such herbs as Buchu (Agathosma Betulina), Burdock, Echinacea and Yellow Dock Root (Rumex Crispus) are supposed to help the kidneys to detoxify. I am not certain whether there is medical proof for this.

Conclusion

Detoxification starts with adopting healthy food and lifestyle habits first. A low carb, low fat type diet (zone diet, a Mediterranean diet or South Beach diet) are essential. They are the footing from which the other detoxification methods work. Some of the toxins will leave you when you work out and sweat. More will leave you when you enjoy an Epsom salt bath. This gives you some extra magnesium, which in turn detoxifies your system. The detoxification takes place  through several biochemical reactions where magnesium is a cofactor. Colonics or enemas will help remove toxins through your colon and they are more effective when coffee enemas are used (stimulation of your liver through the splanchnic nerves).

Infrared saunas

Infrared saunas can help to mobilize toxins and detoxify them through your skin. Take ample amounts of fluids as this will help remove some toxins through the kidneys and the urinary system. If the physician detects major heavy metal poisoning, the traditional intravenous chelation treatments are more recommendable. A naturopathic physician or A4M physician (anti-aging physician) could advise you what is best in your case. Overall the thought is that by removing toxins the mitochondria in our cells will work more efficiently. They are particularly concentrated in the brain, heart, liver, and kidneys. When they work more effectively they give you more energy and prevent major diseases. These diseases are cancer, cardiovascular disease (heart attacks and strokes), diabetes, arthritis and loss of cognitive function. The latter prevents Alzheimer’s disease and dementia.

On World Health Day 2013 the Focus is on Hypertension

Introduction

This article is about this: on World Health Day 2013 the focus is on hypertension. In the US high blood pressure causes 348,000 American deaths per year, in the world its death toll amounted to 9.4 million every year. This is unfortunate as high blood pressure is an illness, which can both be effectively treated and prevented. Uncontrolled high blood pressure (hypertension) causes heart attacks and strokes, can cause kidney failure, heart failure and blindness. You control blood pressure with lifestyle changes and/or medication,  and these risks go away.

The age standardized death rate  (Ref. 1) for both sexes in the US for heart disease is 80.5 per 100,000 and for strokes 25.4. In Canada these rates are 66.2 and 22.9. For  Germany the rates are 75.0 and 31.2, in Italy 51.7 and 34.9 and for Japan 31.2 and 36.7. The death rates from cardiovascular disease per 100,000 people in the same countries is as follows: in the US 172.2, in Canada 130.7, in Germany 200.2, in Italy 153.5 and in Japan 107.1.

There are obviously significant differences in these countries, which I will discuss further below.

Topic of high blood pressure

World Health Day was celebrated on 7 April 2013 to commemorate the founding of the WHO in 1948. In 2013 the topic of high blood pressure made the World Health Organization edit a PDF publication of 155 pages. It is entitled “Global Atlas on cardiovascular disease prevention and control” (Ref.1, be patient, loads slowly). In it prevention and treatment for high blood pressure are discussed in detail. This text points out that there has been a remarkable decline in death rates from heart attacks and strokes (collectively called “cardiovascular disease”) between 1981 and 2000 in the United Kingdom.

A thorough analysis of this showed that 58% of this decline was due to risk factor reduction in the whole population. This included reduction of smoking and heavy alcohol consumption, reduced salt intake, combatting physical inactivity and reduction of saturated fat intake. The other 42% of the decline in cardiovascular disease is due to treatment by a physician. So, it is clear from this that the majority of mortality prevention comes from the patient, less than 50% comes from the treating physician. However, it is important that physicians will educate their patients to cut out risk factors themselves in order to prevent hypertension.

Risk factors for high blood pressure

On World Health Day 2013 the focus is on hypertension. In the past it was thought that most cases of high blood pressure would be due to “essential hypertension”, a term saying “we don’t know what causes high blood pressure”. Many physicians still use this term. Physicians thought that only a small amount of cases were due to “secondary” hypertension with an apparent cause (e.g. kidney disease, hormonal imbalance, pregnancy). But in the meantime research by Harvard University and other research institutions has shown that there are a number of specific causes that contribute to high blood pressure, either alone or in combination.

Common causes of high blood pressure

Here are the commonly known causes: too much salt in our diet; we tend to not eat enough vegetables and salads; we like to sit in cars, in front of the TV or in front of the computer (physical inactivity). Many people still smoke, although tobacco is known to cause high blood pressure and lung cancer. Too much alcohol is known to cause hypertension as well. So the following steps will prevent high blood pressure:

  1. consuming less salt
  2. eating a balanced diet (preferably the DASH diet)
  3. engaging in regular physical activity
  4. avoiding tobacco use
  5. avoiding harmful use of alcohol (more than 2 oz. or 60 Grams per day)

Diabetes worsens the risk for heart attacks and strokes

Physicians know that diabetes worsens the risk for heart attacks and strokes and increases the risk of high blood pressure as well. So, some hidden risk factors for high blood pressure related to diabetes are as follows: a high fasting blood sugar; obesity; food with too much fat, too much sugar and too many starches (not enough complex carbohydrates).

What can we do to reduce death rates from high blood pressure?

As Canada is one of the countries where the death rate from strokes and heart attacks is lower than in the US or Germany, I like to point out some of the reasons for this. I practiced medicine in Canada for many years. The “Canadian Hypertension Education Program” have been guidelines for practicing physicians to follow providing effective screening and treatment of high blood pressure. Cardiologists at various continuing education conferences have promoted this.

High blood pressure recalls at my medical office

At my office I had a hypertension recall program where my staff called every patient with high blood pressure into the office every 3 months. We would review the home-measured blood pressure readings from the patient (recorded in a little booklet). I also took the blood pressure of the patient and so did my staff on the patient’s arrival. We reviewed the blood pressure medication and reviewed the possible side effects. I explained to the patient what to do, if the blood pressure was higher than normal (possible adjustments of the medication at home). I also encouraged my patients with regard to the life style issues (the 5 points mentioned above). Over the years the number of patients who developed heart attacks or strokes declined, as one would expect.

Literature review in the medical journal Canadian Family Physician

A recent review in the Canadian Family Physician mentions that there is room for improvement regarding the Canadian statistics. As mentioned above Italy and Japan are doing better with regard to mortality from heart attacks and strokes compared to Canada. We have a health care system in Canada that is available to every Canadian resident and funded by provincial taxes. In this system patients do not have to pay for office visits (although they pay for it indirectly through taxes). For the patient with high blood pressure, it means that there is a system in place, which helps prevent cardiovascular disease and treats high blood pressure effectively. In my opinion the home recording of self-measured blood pressure readings at least once per day with a home blood measure monitor is vital to encourage the patient to be engaged with regard to his/her blood pressure problem.

Newer findings about high blood pressure

For years physicians did not know where high blood pressure came from. In the last few years research has shown that nitric oxide plays an important role in preventing high blood pressure. The lining of your arteries produce nitric oxide (by the so-called “endothelial cells”). This is the natural artery relaxer.

Foods that produce nitric oxide in the body are spinach, kale, red beet, cabbage varieties and other vegetable greens. These foods, which are also contained in the DASH diet, and regular exercise will stimulate the lining of your arteries to produce nitric oxide, which prevents high blood pressure, heart attacks and strokes.

Treating high blood pressure with medication

If these measures and recommendations to prevent high blood pressure do not help, it is time to treat it. It is important that the patient who needs high blood pressure treatment with medication, takes the medicine regularly. This has the name”compliance”. By keeping the blood pressure reading below 120/80 you prevent your risk of getting a heart attack, a stroke, heart failure or blindness from broken retinal vessels. If the patient develops any side effect from the medication, it is important to see the physician about this right away. It may be that the medication has to be adjusted or altered.

Nitric oxide

Nitric oxide can be taken as a supplement (Neo40), which allows the endothelial lining to be regenerated as indicated in this interview with the inventor, Dr. Nathan Bryan from the University of Texas Health Center in Houston.

The older we are, the more likely it is that our blood pressure will be high. As this link shows, 2 out of 3 people above the age of 60 in the US have systolic hypertension (the upper value of the blood pressure is elevated). As we age, it appears that the lining of the arteries do no longer produce the required amount of NO (nitric oxide). NO is necessary to prevent high blood pressure and prevent hardening of the arteries.

Adopt the Mediterranean diet 

So, it would be wise to adopt the Mediterranean diet. This includes lots of vegetables, spinach, kale, bok choy, Swiss chard and others to boost your NO production. You still measure your blood pressure regularly. If you do not have a home blood pressure monitor, go to a pharmacy that allows you to check your blood pressure for free. If it is above 120 over 80 seek the advice of a health professional. You can find more information in Ref. 1.

In essence, what World Health Day 2013 asks us to do is to pay attention to your blood pressure and make sure it is normal.

My Experience With Cancer Research

This article is about my experience with cancer research. April is cancer awareness and fundraising month. I thought it would be interesting to analyze what’s going on behind the scenes of cancer research. I was a cancer researcher for over 3 years at the Ontario Cancer Institute (OCI) from 1972 to 1975 and I will share some insider experiences here.

1. Publish or perish

Our supervisors said: “publish or perish”. In other words all the experiments we did needed to fit into the larger picture the group was working on. And the results should be different, interesting and most of all publishable. There had to be significant differences between experimental groups and controls. This was a requirement by publishers of medical journals would accept them for publication. There were often two or three manuscript revisions where the content was “massaged”.  I had to pay attention to proper wording and comparing or opposing the results with other publications. This way the publisher deemed the manuscript “publishable”.

2. Fund raising awareness

One of the major fund sources for cancer research in Canada was the MRC (Medical Research Council of Canada), which has been replaced by the Canadian Institutes of Health Research (CIHR) in 2000. Without money there is no cancer research, so everybody was aware of the policies and expectations of the fund source.

3. Mouse model versus human tissue based research

My work was in the immunology section of the biophysics department, where researchers perform basic medical research at the OCI. In this department much research had already been performed separating cell populations in a mouse model to determine what cell types were needed to initiate an immune response. The B cells in mammals are antibody-producing cells of the immune system that protect from viruses. T cells are lymphocytes, which the body processed in the thymus. They turn into killer cells, which can attack parasites and also cancer cells. I was working in this area.

Cell separation experiments and doubts about mouse research

We did cell separation experiments. With this method I was able to separate cells according to cell size and collect them in vials. Subsequently I did remix experiments. The purpose was to find out which cell types were able to mount an immune response. My supervisor suggested the use of a mouse tumor cell line as targets. I started questioning whether a mouse model would be the appropriate model to study human cancer biology. But my superiors were not in agreement. The “holy grail” was that to work in a mouse model (mouse mammalian cells).  The assumption that this should also work in the human situation (human mammalian cells).

4. Non-medical researchers in cancer research

This is a thorny issue, but a reality. My immediate supervisor in cancer research had a PHD in physics. His physics degree was perfect for sorting out density issues for cell separation experiments. The co-chair of the immunology department had a PHD in biology. His qualification was good for conducting mouse experiments. Both of them felt somewhat insecure when I asked questions. I wanted to know how relevant mouse experiments were for the examination of human cancer conditions. As I needed to publish my experiments, I had to quiet down and concentrate on the mouse model the team was working on. For a while this could even be exciting as we were studying the cell interaction between macrophages and T cells to mount a cell-mediated immune response.

5. Regulation of the cancer research industry

After playing with cell cultures for 2 ½ years it was time for me to reach out to get a job in the cancer research field or else go back to medicine. In1975 there was no equal opportunity legislation in place that would have protected me as a landed immigrant from discrimination. The reality in 1975 was that only Canadian born physicians who attended a Medical School in Canada could become a director of a cancer research facility in Canada. The rules for me (I had left Germany right after my rotating internship) were that I had to go through further medical training and pass the Canadian licensing exam (LMCC), which I did eventually at McMaster University in Hamilton, Ontario.

Interview with Dr. McCulloch

One final attempt to shed light on my options was an interview with the “big boss” at the Ontario Cancer Institute at the time, a physician cancer researcher, Dr. Ernest A. McCulloch, for whom I had great respect. He was a sharp thinker and had vision, and he was a fellow physician. I asked him what he would do on the long-term, if he was in my place. He said that in the long-term with my medical background it would be a lot more satisfying for me to get back into medicine and practice medicine. However, he wanted me to go on for another 1 or 2 years and publish more papers together with my supervisors.

My decision to leave cancer research

I decided for myself right there that I would leave cancer research and I prepared quietly for my exit. Within a short time I got a position to work as an intern at a hospital at McMaster University and in the spring of 1978 I passed the LMCC (licensing) exam. As a fully licensed physician in Canada, I was no longer interested in “slave work” in cancer research. I also left the cold winters of Ontario behind and went to the west, to British Columbia.

6. Future vision of medical cancer research

Research has come a long way. Recently I came across a new breast cancer protocol, which is non-toxic, without chemotherapy and without radiation. It is so unconventional that the US research team, aware of the politics in the US, decided to do the initial trials in the Caribbean. I wrote a blog about this new breast cancer treatment protocol, which I believe will become the future standard for breast cancer therapy and perhaps even for other cancers.

Alternative breast cancer treatment

In Germany and Switzerland an alternative breast cancer treatment consists of with mistletoe extracts. This is a non-toxic plant chemotherapy, which slows down tumor growth. It has a dual action, namely a chemotherapeutic effect, but at the same time an immune system stimulating effect. Here is a study going back to 2001, which is still relevant. There was a 40% long-term survival benefit in the Iscador group when compared to a control group without treatment. Normally, oncologists jump at such an excellent chemotherapeutic agent as they even consider chemotherapeutic agents that with a 25% beneficial survival effect a good treatment option. However, as the medication is a simple mistletoe extract and cannot be patented, Big Pharma is not interested in marketing this. As a result cancer treatment protocols in Europe are significantly different from those in North America.

The future of cancer treatments

In the future I would expect that non-toxic treatment methods for any type of cancer will be more successful than any chemotherapeutic or radiation treatment approaches as both interfere with the immune function, which is what will kill the patient at the end. Cancer is a disease where the immune system fails. So, cancer patients need teaching how to stimulate their immune system. This is the only thing that controls cancer on the long-term.

You will hear more about epigenetic switches as often a cancer producing substance will turn off a gene (epigenetic switch) and this causes cancer.  Remove what throws that switch into the off position or introduce a healing agent that resets the switch and the cancer will get eliminated.

7. Prevention of cancer

Researchers noticed that herbs, spices, vitamins and minerals contain the most powerful cancer preventatives. Did you know that curcumin, derived from the Indian spice turmeric, prevents a number of cancers? Similarly, vitamin D3 at high enough doses (4000 to 5000 IU per day) has been shown to prevent cancers. Linus Pauling showed long time ago that vitamin C at high enough dose would be an antioxidant and would stimulate the immune system and thereby be a cancer preventative. It works together with a detoxifying antioxidant, glutathione in the liver to neutralize any free radicals, which are aggressive chemicals that cause cancer.

Learning from risk factors for cancer

There are many other vitamins and minerals that I have explained elsewhere, which are needed together with organic food to give you the building blocks for a stable cell metabolism. This in turn will strengthen the immune system to defend you from toxins of the environment. A simple step like a daily exercise routine can cut your cancer risk down to 50% compared to those who elect to not exercise. Did I mention the importance of quitting smoking and cutting out alcohol? The “quit smoking” part has been known for some time.

Alcohol is a cell poison and can cause cancer

At the Anti-Aging conference in Las Vegas in December 2011 I learnt about the importance of alcohol exposure that causes cancer. Even smaller doses of alcoholic beverages over a long period of time can cause cancer. First I thought it would be a big deal to quit alcohol entirely. But since I have quit the modest few drinks per month, I actually have not missed them at all! I strongly believe in cancer prevention, so quitting alcohol completely was only one small step in this overall objective. In view of the recent statement by the WHO that 70% of all deaths are caused by smoking and drinking of alcoholic beverages, it behooves us to change our lifestyles, if we are at all interested in a healthy long life.

Conclusion

Nothing has changed in cancer research circles since the time when I was part of it. Basic cancer research involving animal experiments is necessary. But in my opinion cancer research should be more human-centered using human cell lines in culture and using clinical trials. Ultimately cancer research needs to invent and develop new non-toxic cancer therapies to cure cancer patients.
