Hair Style Products are Mostly Safe

A recent study concluded that hair style products are mostly safe when it comes to causing cancer. In the past there was some concern that the dyes for hair coloring could cause cancer. In a previous blog I mentioned that hair stylists are at a greater risk for developing bladder cancer.  But in customers who had their hair colored this was more difficult to prove. The new study reviewed by CNN was based on a large cohort of 117,200 female nurses at Brigham and Women’s Hospital in Boston.

These 30-55-year-old nurses were observed for 36 years. Detailed information about hair coloring was part of the study.

Results of the hair coloring study

Women with light and medium use of hair dyes did not develop non-melanoma skin cancers, hematopoietic cancers or squamous skin cancers. The same was true for bladder cancer, melanoma, estrogen receptor positive breast cancer, progesterone receptor positive breast cancer and hormone receptor positive breast cancer. Furthermore hair coloring did not cause brain cancer, colorectal cancer, kidney cancer and lung cancer. 5% of women who ever used hair dyes did develop basal cell skin cancer; there was a total of 22,560 basal cell cancers that occurred.

The BJM showed that women who colored their hair regularly (termed “cumulative dose”) developed 24%-31% more breast cancer. This breast cancer was estrogen negative breast cancer (31%) and progesterone negative breast cancer (24%). They also developed 24% more ovarian cancers and 17% colorectal cancer than controls who did not dye their hair. Myeloid leukemias were also more common in the heavy use hair coloring group with 29% and follicular lymphomas with 13%. Other forms of leukemia were not more frequent.

Discussion

50% to 80% of women and about 10% of men ages 40 and older are using hair dye regularly in the United States and Europe. The above cited cancer frequencies were based on women with dark hair and a cumulative dose of more than 200 applications of permanent hair dye. Translated into years of application of hair dye this means coloring your hair for 16 to 17 years, if you color your hair once a month. The study is very powerful due to the large number of women examined and the amazingly long time of 36 years of observation.

Heavy use, medium and light use of hair dyes

It is interesting to note that only the heavy use of hair dye resulted in a few significant cancer figures. The light use (1 to 99 times) and medium use (100 to 199 times) of hair dyes did hardly lead to any cancer. There were some exceptions where light use of hair dye led to 34% more bladder cancer and to 67% more follicular lymphomas. These were oddities, because the lighter use of hair dyes led to higher amounts of these cancers than medium and heavy use. This is not what the researchers expected.

Hodgkin’s lymphoma and breast cancer

Hodgkin’s lymphoma was about 3- to 5-fold more common in all hair coloring categories compared to non-use of hair dye. The authors discussed subgroups of women as well. They noticed that black women using permanent hair dye had a higher risk to develop breast cancer than white women who only had a borderline increased risk. Estrogen negative, progesterone negative and hormone negative breast cancer was more likely to be caused by heavy use of hair dyes.

No association between the following cancers and hair dye use

The present study could not find an association between ever using hair dye and the following. The study found no association with cutaneous squamous cell carcinoma, melanoma, ovarian cancer and colorectal cancer. In addition there also was no association with  kidney cancer, lung cancer, and brain cancer. But it found a slight increased risk (5%) of basal cell carcinoma.

The study contains no information regarding the specific ingredients in hair dye that are carcinogenic. In the US supervision of permanent hair dyes by the FDA is rather loose. This leaves the manufacturer mostly in charge regarding the composition of the hair dyes. This is different in Europe and in Japan. Here manufacturers removed several carcinogenic substances from hair dyes because of regulations by the local regulatory bodies. This could have a cancer-lowering effect in these countries.

Conclusion

This large study from the British Medical Journal, published Sept. 2, 2020 explains the relationship of cancer risk to permanent hair dyes. The study lasted 36 years and involved 117,200 female nurses. More than 96% of the women were Caucasians. This study showed that for most people who use permanent use hair dyes there is no association with most cancers. However, there was an association between a few cancers and the use of hair dyes. These were in 5% basal cell skin cancers. But breast cancer (31% estrogen receptor negative, 24% progesterone receptor negative), ovarian cancer (24%) and 17% colorectal cancer were among these too. There was also an increased risk of Hodgkin’s lymphoma.

Effect of screening by FDA regarding carcinogenic substances in hair dyes

All this data was based on the hair dyes used in the US. Supervision of permanent hair dyes by the FDA is rather loose, with the manufacturer being mostly in charge. This is different in Europe and in Japan. Here local regulatory bodies instructed manufacturers to remove several carcinogenic substances from hair dyes. This could have a cancer-lowering effect in these countries.

Pollution And Soaring Lung Cancer Rates

In early 1900 lung cancer was unheard of. This was before the cigarette industry started to mass-produce and market cigarettes.

However, ever since the arrival of the industrial revolution air quality has suffered. In China poor air quality has now reached such enormous values that the specialized cancer agency of the World Health Organization, the International Agency for Research on Cancer (IARC) has labeled poor air quality as one of the causes of lung cancer.

When you rank countries by average air pollution measurements, one sees that Europe, the US and South America overall have good ratings, whereas the Middle Eastern countries, China and India have poorer ratings.

However, when the pollution index of cities where the population is much denser than in the countries at large, are tabulated a much different picture emerges: Cities in Iran, India and Pakistan stand out as particularly bad followed by cities in China, Eastern Europe, Paris, London, Berlin, cities in California (the populous State), Chicago and New York.

Pollution does not stay local, but travels through the stratosphere around the globe. The result is that now 10 to 15% of lung cancer in the US occurs in patients who never smoked. This translates into 16,000 to 24,000 deaths annually of never-smokers in the US.

In certain cities such as Beijing the lung cancer rates have doubled in 9 years between 2002 and 2011. As this article shows lung cancer in never smokers can be caused from exposure to radon, to second-hand tobacco smoke, and other indoor air pollutants can also cause such cancers. But the outdoor air quality has been a problem ever since the industrial revolution, which started around Europe in the 1800’s and first part of the1900’s. In the latter half of the 1900’s much of the industrial wave has migrated to the Middle East, to India and China. But the air quality of the whole world has suffered as the jet stream and other air currents carry pollution in the stratosphere all around the globe.

History of pollution in various regions

1. In Germany’s  Ruhr district (“Ruhrgebiet”) in North Rhine-Westphalia, a highly populated industrial area, pollution reached a peak in the late 1950’s. From 1963 onward many of the coal mines, iron ore mines and other mineral mines closed down. 50 years ago the German Chancellor, Willy Brand was concerned about the environment and promised that blue skies would return to the Ruhr district again.  A special task force was initiated and maximally allowable limits were established for industries’ pollution emissions and enforced by the German government. Government and industry were co-operating in developing anti-pollution measures, which have cleared up a lot of the pollution since. With regard to car emissions lead free gasoline was introduced and carburetors ensured more complete burning of exhaust gases. This is now common and accepted anywhere except for diesel fume exhaust, which nobody wants to address despite proven carcinogenicity.

Now Germany is one of the leaders in green technology, which is also important for tourism.

2. England has its own legacy of pollution in soil and air from the industrial revolution. The soil of moorland, which soaked up acid rain for decades, is more acidy than lemon juice and it will take a long time despite industrial complexes having closed long time ago, before the soil quality will be returned to normal.

3. Hamilton in Ontario/Canada has had a longstanding pollution problem, which I witnessed from 1976 until my departure in 1978. It is well known that Stelco, the local steel plant downtown Hamilton is sending polluting emissions into the air. In 1976 a vising professor from Australia gave an interesting talk about a study that was done at that time regarding the risk of developing bronchogenic carcinoma (a synonym for lung cancer) in the immediate surroundings of the Stelco plant. He said that this was one of the first studies to show that the distance of people’s houses from the source of pollution mattered as that determined how concentrated the air pollution was (the closer the more polluted the air). This  affected cancer rates: they were much higher in the immediate surrounding of Stelco when compared to the average rate in the rest of Hamilton. This difference was very significant within a radius of 1 kilometer (= 0.62 miles) from the Stelco plant.

Just in May of 2013 the local cancer agency of Hamilton announced that the lung cancer rate in Hamilton was higher than elsewhere in Ontario because of a combination of poor air quality and of a higher percentage of people smoking. Then in August 2013 the city of Hamilton announced a new air pollution bylaw for stricter pollution measures to improve the air quality in the downtown area. It is just a pity that Hamiltonians had to wait until 2013 before the city approved an anti-pollution bylaw that could have been passed 50 years earlier like in Germany’s Ruhr district!

4. In 2008 Pittsburg, a former steel manufacturer town like Hamilton, Ont. outdid Los Angeles with regard to small particle air pollution.

Lung cancer prevention by the authorities

As mentioned before up o15% of lung cancer is caused by environmental exposure. So, we ourselves can only prevent 85% of lung cancer by not smoking and not exposing ourselves to industrial emissions or to smoke from incense. However, in many cities around the world you will get exposed to air pollutants that are well above the safe limits, so the risk of getting lung cancer from just breathing the air there can be much higher than in rural areas where there is no industry.

Technologies to control air pollution are widely available. We need to exert pressure on politicians to show leadership around the world. Government regulations to lower emission rates need to be put into place and inspectors need to ensure the rules and regulations are adhered to. Without reducing emissions of cancer producing gases and chemicals right at the source (open burning of cuttings in orchards or burning cut trees), cutting emissions of cars, planes, ships, diesel cars, locomotives, electric generator plants etc. the air quality will not improve. Despite some costs involved industry, governments and individuals have to work together to make clean air happen.

The residents of those countries that have low pollution values will not benefit, if pollution continues to occur in other parts of the world as it just travels in the stratosphere around the globe until it arrives right here at home! We need an international pollution police. Satellites can be used to monitor where pollution occurs and this can be followed up through the local regulatory bodies with penalties and remedial actions.

What can I do personally to prevent lung cancer?

1.The most obvious step is to quit smoking and ask smokers who come to your place to smoke outside (not in your home).

2.Consider moving away from the city, if the air quality is unacceptable to a place where there is low air pollution.

3.Vitamin D3 has been shown to prevent colorectal cancer, but as there are vitamin D receptors found on the surface of various cells in tissue around the body including the lungs, many researchers feel that this vitamin in higher doses (2000 IU to 5000 IU) has probably a wider applicability in preventing cancers, even lung cancer.

4.Cutting out sugar and adopting a Mediterranean type diet is a prudent thing to do; also cutting down your calories to the maintenance you need (mildly ketogenic diet). If you bought body composition scales, it would display what your daily calorie consumption is and you should not exceed this, or else you’ll gain weight. An aging man who is overweight will experience hormone changes as fat is being metabolized and the enzyme aromatase contained in fatty tissue will turn male hormones (testosterone, DHT, androstenedione) into estrogen. Estrogen (particularly estradiol) is a known carcinogen that has been proven to cause breast cancer in women and prostate cancer in men. However lung cancer is also being promoted in women by estrogen as discussed in this link. In men one needs to remember that lung cells have estrogen receptors and there is concern in aging men with higher estradiol levels that this can promote cell divisions in existing lung cancer. So, it is important to maintain a normal body mass index between 21 and 24 (well below 25.0 and well above 18.5, which are the official accepted limits). This way there is no problem with insulin resistance (too high an insulin level), and other metabolic substances (cytokines, growth hormone like factors and tumor necrosis factor-alpha from body fat) that are cancer promoting.

5. If testosterone deficiency is present, which is common in older men, testosterone will have to be replaced with bioidentical hormones. It is a myth that testosterone would cause prostate cancer. Testosterone in males is necessary to maintain a normal metabolism including the immune system, which then can fight lung cancer and any other cancers.

6. Exercise and reducing beef consumption are also often mentioned in terms of preventing lung cancer.

7. Here are several recommendations from the LifeExtension Foundation that I found very useful in terms of lung cancer prevention. This link shows that antioxidant vitamins such as vitamin C, alpha tocopherol, the minerals selenium and zinc are also helping to reduce the lung cancer rate. Drinking green tea has also been shown to be effective in a dose-response curve manner (more tea protecting more from lung cancer). Vitamin B12 and folate have been shown to reduce abnormal bronchial cell growth in smokers as shown by repeat bronchoscopy studies.

8. Those who have been smokers in the past and those who have been around heavy smokers for more than 10 years in the past should consider having a preventative bronchoscopy done by a lung specialist (also called respirologist or pulmonologists). This way any suspicious areas with precancerous lesions can be biopsied during the procedure and attended to.

Hopeful research for new lung cancer treatments

Lung cancer is a disease that is best prevented. Once a person gets lung cancer, the prognosis is still very poor. However, cancer researchers are getting close to newer treatments involving genetically modified T-cells (killer cells) as was recently achieved for leukemia. Similar research is going on regarding ovarian cancer, melanoma, lung cancer and pancreatic cancer.

Conclusion

It is not acceptable to let pollution take its course , the way politicians around the globe have handled this in the past 6 decades with a few notable exceptions mentioned. We all suffer a higher risk of getting lung cancer, even if we have been life-long non-smokers. Right now up to 15% of lung cancer in most populations are of this type. However, in Beijing this number is already much higher. The technology is available; Germany has led the way in the Ruhr district in the 1960’s and beyond. In my opinion the G8 meetings should have this high on their agendas and send technological aid to all the regions that have higher than the average world pollution index under the mandate of a special UN commission. This should be supported by the major industrial players with the knowledge that they will prevent the death of millions of potential consumers down the road, which will on the long-term pay off the relatively minor investment of installing pollution controls, before lung cancer levels rise even more.

Lycopene Benefits Backed By Science

Lately a lot of attention has been directed to the health benefits of vegetables and fruit. Vitamin C has long been an accepted household term, and nobody questions the benefits. Newer buzz words are the terms “bioflavonoids” and “antioxidants”. Some products are aggressively marketed extolling the above named beneficial substances, but often the consumer is left mildly bewildered by exaggerated claims. Often the sale prices of these miracle foods are as lofty as the bold statements that go along with them.
For any shopper it is important to know that some of the most beneficial foods are not high priced items, but very common staples. Take tomatoes, for instance. They are a significant source for the substance lycopene, which lately has received a lot of attention. Lycopene and its dietary sources as well as its benefits have been researched world wide, and the results are now in. It is responsible for the red color in fruit or vegetables, such as tomatoes, and its isomeric form 5-cis-lycopene is the most stable form having the highest antioxidant properties. Common dietary sources are tomatoes, watermelons, pink guava, pink grapefruit, papaya, apricot and other fruit. In the Western diet tomato-based foods account for about 85% of dietary sources of Lycopene. Studies have shown that lycopene is more efficiently absorbed from processed tomato products compared to raw tomatoes. Once it is absorbed it is distributed throughout the body. The highest levels showed up in the testes, the adrenal glands, prostate, breast and liver.
Research going back to 1995 showed an inverse relationship between the consumption of tomatoes and the risk of prostate cancer. A follow up publication in 1999 showed that the same inverse relation of lycopene intake and cancer also included breast, cervical, ovarian, liver and other organ sites. Further studies have followed these initial publications, and the great majority of them suggest that an increased intake of lycopene showed an association with a significant reduction in the risk of many cancers.
Coronary heart disease and lycopene benefits were also examined. The strongest population based evidence comes from a multi center case control study in Europe (EURAMIC). 662 Cases and 717 controls were recruited from 10 different European countries, and there was a significant relationship between levels of lycopene in fatty tissue and the risk of myocardial infarction. Lower lycopene levels were associated with a higher risk of heart attacks.Lycopene was also shown to decrease levels of oxidized LDL (LDL or low density lipoprotein is known as the “bad” cholesterol). Another small study showed that lycopene was reducing total cholesterol levels and as a result was lowering the risk of coronary heart disease (CHD).
The list of benefits does not end here: the dietary oxidant reduces oxidative stress and levels of bone turnover markers, meaning that it may contribute to the bone health, especially reducing the risk of osteoporosis in postmenopausal women.

For people with mild hypertension (high blood pressure), consumption of lycopene resulted in significant reductions of systolic and diastolic blood pressures.
Infertility in males was significantly helped by lycopene intake. In a study infertile man received 8 mg lycopene per day in capsule form. Laboratory tests confirmed an increased sperm density along with functional sperm concentration and mobility. This treatment protocol with lycopene supplementation resulted in a success rate of 36% pregnancies in their partners.
Pregnant women with pre-eclampsia who were treated with lycopene supplement significantly improved, which was shown by decreased diastolic blood pressure, the reduction of pre-eclampsia and a decrease of intrauterine growth retardation, resulting in a healthier mother and baby.
Future research is pending surrounding lycopene in metabolic and inflammatory diseases and in its role of possibly preventing neurodegenerative diseases such as Alzheimer’s disease. Other inflammatory conditions such as arthritis and emphysema will likely also be shown to benefit from lycopene. Preliminary data has already indicated this.
The Food and Drug Administration (FDA) of USA has recently approved lycopene as a safe “natural coloring agent” and a Generally Recognized as a Safe (GRAS) component. The Department of Nutritional Sciences , Faculty of Medicine, University of Toronto, c/o Dr. A.V. Rao et al. who completed this meta analysis of the recent literature have recommended that we all consume a regular daily lycopene dose in our food and supplements as part of our diet for good health.

Ginger Kills Ovarian Cancer

Active substances in ginger are killing cancer cells in lab experiments. This is the gist of laboratory studies led by Dr. J. Rebecca Liu at the University of Michigan Medical School. The findings on these studies were presented at the annual meeting of the American Association for Cancer Research.
In the past it has been implied that the substances (6)-gingerol and (6)-paradol interfere with carcinogenic processes. Gingerol also has been linked with antioxidant and anti-inflammatory properties.
The research group from Ann Arbor used whole ginger extract which was standardized to a contents of 5% (6)-gingerol to explore the death of ovarian cancer cells in vitro. They suggest that ginger can circumvent chemoresistance.

Due to this quality ginger substances can be effective in cancer cells that have been resistant to chemotherapy.

More Education Needed About Ovarian Cancer

Campaigns that inform about breast cancer are abundant in the media, but the one cancer which may be the most fatal of gynecologic cancers is silently at work, killing more than 60% of women diagnosed – ovarian cancer.
It is also the cancer women know shockingly little about, and most women cannot identify its symptoms. Dr. Barbara Vanderhyden from the University of Ottawa, Canada confirmed in a study, that there are a lot of misconceptions about the disease. One in three women falsely believe that a Pap test screens for ovarian cancer. Remarkably, 12% of Canadian women claim that they have never heard about the disease, and only 35% consider themselves well informed. Even more problematically, women 50 years and older-those with the highest risk for ovarian cancer- are significantly less likely to be aware of their risk for disease than their younger counterparts. 71% of women think that ovarian cancer mainly affects women under 50 years of age.
In the view of the findings that 96% of the women could not identify a combination of the most common symptoms of ovarian cancer, it is of great importance that more education and information is needed. Women as well as their health care providers have to be vigilant about early warning signs and symptoms. The most common warning signs and symptoms for ovarian cancer are bloating, abdominal pain, changes in urinary frequency, weight fluctuation and nausea.

Diagnostic tests involve a bimanual examination by a physician, which is needed for all women, including those who did have a previous hysterectomy. The other test to consider would be an abdominal ultrasound.

Beware Of Binges

In the June 16, 2003 issue of Time (page 73) an article appeared under the heading “Summertime booze”. A study from the Buffalo University showed that women, in particular, have to be conscious that drinking 3 or 4 drinks at one time on the weekend is not the same as drinking the same 3 to 4 drinks over the course of one week.

A study looking at the frequency of breast cancer in relation to drinking patterns found that the binge drinking women had a 80% higher breast cancer risk than the controls who drank 3 to 4 drinks at a time (but only one drink per day). Jo Freudenheim, the epidemiologist involved in this study, suggested that perhaps with the binge drinking alcohol’s toxic potential for breast cancer cells had been reached whereas with one drink at a time over one week this level was never encountered.

In another study from the Buffalo University liver toxicity as a result of alcohol exposure was examined. These researchers used blood tests to measure liver enzymes, which were leaking from the liver cells as a result of the toxic effect of alcohol. Both men and women were tested and various drinking patterns were also studied. Men who drink several drinks daily had the highest liver enzyme counts (reflecting the toxic effect of alcohol on the liver). However, women who drink only on weekends had even higher counts of the liver enzymes than men! Women who drank on an empty stomach had much higher liver enzymes in these studies than men where this effect did not show (with men it did not matter whether they drank with food or on an empty stomach).

The researchers concluded that binge drinking appears to affect women more than men. Generally speaking the higher the amount of alcohol consumed, the more toxic the effects on body cells and on the liver. Moderation may be in order.

Comments: These type of studies are particularly important in view of the fact of marketing techniques of the wine industry. Wine and alcoholic beverages are being portrayed as being a good source of bioflavonoids that are lowering cholesterol and would prevent or postpone heart attacks. Some of the data on cancer indicates that for ovarian cancer and colorectal cancer there may not be a safe low dose as even one drink per day can have a measurable effect on cancer risk. On the other hand, bioflavonoids are abundantly present in raw vegetables and fruit, so there is no panic about not getting enough heart attack preventing foods. Finally, a bit of common sense does not harm: alcohol is a cell toxin, so it should be diluted (nothing stronger than wine) and if you desire a drink, use it in moderation.

Hyperthermic Chemotherapy For Colon Cancer

When colon cancer comes back (Duke stage D cancer) there often are seedlings of cancer cells in the peritoneal cavity, which in the past made surgery impossible. However, at Montreal’s Maisonneuve-Rosemont Hospital the surgical oncologist, Dr. Pierre Dubé, is using a brand new technique that allows to do surgery even in these difficult cases. It has been known for some time that cancer cells are heat sensitive.

At the time of the surgery all of the visible cancer is removed, like with ovarian cancer, using a cytoreductive surgery approach. At the end all invisible cancer cells are treated with a combination chemotherapy fluid that is introduced into the abdominal cavity and that is heated in a controlled manner. Heat probes measure the temperature and make sure the fluid is heated to 44°C (111°F). Other experiments have shown that at this temperature chemotherapeutic agents enter into the cancer cells faster, as they had been rendered more fragile by the heat. The end result is an increase in survival by 2 to 2 1/2 years. This is a breakthrough because in the past conventional combination chemotherapy for this group of advanced colorectal cancer patients achieved a remission (meaning “response rate”) of 0%. Now with this new technique there is a remission rate of 30% to 50%. Hyperthermic chemotherapy for colon cancer was pioneered at the Gustave Roussy Institute in Paris where Dr.Dubé was introduced to this technique. This was reported in the Jan. 14, 2003 edition of The Medical Post (page 7). According to Dr.Dubé about 10% of all colorectal cancer patients will need this type of surgical approach and about up to 25% of patients who develop cancer spread into the abdominal cavity (peritoneal carcinomatosis) would be potential candidates for this treatment modality.

Comments: Recently there has been a paper intitled “Factors predicting survival after intraperitoneal hyperthermic chemotherapy with mitomycin C after cytoreductive surgery for patients with peritoneal carcinomatosis” by P.Shen et al. (Arch Surg – 01-JAN-2003; 138(1): 26-33). 109 patients with carcinomatosis of the abdominal cavity with different cancers were treated with cytoreductive surgery and hyperthermic chemotherapeutic solutions.
